Treatment-Resistant Depression

Abstract: Our ability to treat depression has improved with the availability of receptor-specific and chemically diverse groups of antidepressants. Even now, most of the short-term studies indicate that about 20% of depressed patients remain resistant to treatment. Therefore, it is important to properly assess the treatment-resistant depressed (TRD) patients and to separate the truly refractory patients from those inadequately treated. “…Definitions have varied widely as treatmentresistant depression encompasses a broad continuum of severity and complexity. In fact, proposed staging criteria for treatment-resistant depression range from failure to respond to an adequate trial (dose and duration) of a single antidepressant to failure to respond to multiple classes of antidepressants, as well as electroconvulsive therapy Rush, 1995, 1997;Ananth, 1998).…”

“…For instance, drug-resistant depression is a clinical problem that occurs in about 20% of depressive episodes [29]and is amenable to cognitive behavioral treatment [30]. Resistance to exposure in panic disorder was found to be associated with lower compliance with regard to exposure homework [31].…”
