Treatment Outcomes and Survival Based on Drug Resistance Patterns in Multidrug-resistant Tuberculosis

Abstract: The revised XDR-TB definition is appropriate for defining patients with MDR-TB with the poorest outcomes. Both pre-XDR-TB(o) and pre-XDR-TB(s) were independently associated with poor long-term survival in patients with MDR-TB. SM susceptibility was linked to better survival in patients with pre-XDR-TB.

“…This again raises concern about the impact of fluoroquinolone resistance on poor treatment outcome. KIM et al [10], in South Korea, Data are presented as n or n (%), unless otherwise stated. XDR: extremely drug resistant; TB: tuberculosis; pre-XDR-TB: multidrug resistant-TB plus quinolone resistance without resistance against injectables.…”

“…In XDR-TB patients, a favourable outcome can vary from 20% to 60% depending upon factors such as fluoroquinolone resistance, previous TB treatment, directly observed therapy throughout, MDR-TB treatment duration and surgical resection [2][3][4]. The key drugs in the management of MDR-TB and XDR-TB patients are newer fluoroquinolones [6][7][8] and second-line injectables [9,10].…”

“…The definition of XDR-TB is resistance to Isoniazid and Rifampicin plus resistance to any fluoroquinolone and at least one of the three injectable second-line drugs used in TB treatment (amikacin, kanamycin, or capreomycin). Pre-XDR is a term given for strains which have a susceptibility in between MDR and XDR with resistance to either Fluoroquinolone or any one of the injectable second line drugs, in addition to INH and Rifampicin [4].…”

“…TB is treatable however drug resistant forms to rifampin as well as multi and extensively drug resistant strains are growing at a significant rate. The success rate of treating MDR and XDR is lower than susceptible forms with a rate of 48% and 29% respectively [1]. In addition the rise in incidence of non-tuberculous mycobacterium in patients who are immunocompromised and in vulnerable populations such as sickle cell disease by mycobacterium avium and mucogenicum infections are reported [9].…”
