This paper gives practical guidelines for diagnosis and treatment of chronic pancreatitis. Statements have been elaborated by working teams of experts, by searching for and analysing the literature, and submitted to a consensus process by using a Delphi modified procedure. The statements report recommendations on clinical and nutritional approach, assessment of pancreatic function, treatment of exocrine pancreatic failure and of secondary diabetes, treatment of pain and prevention of painful relapses. Moreover, the role of endoscopy in approaching pancreatic pain, pancreatic stones, duct narrowing and dilation, and complications was considered. Recommendations for most appropriate use of various imaging techniques and of ultrasound endoscopy are reported. Finally, a group of recommendations are addressed to the surgical treatment, with definition of right indications, timing, most appropriate procedures and techniques in different clinical conditions and targets, and clinical and functional outcomes following surgery. © 2010 Editrice Gastroenterologica Italiana S.r.l

A krónikus pancreatitis a hasnyálmirigy idült gyulladásos megbetegedése, amely a hasnyálmirigy strukturális és funkcionális károsodásával jár. Az esetek jelentős részében fájdalom, emésztési zavarok, testsúlyvesztés áll a tünetek közép-pontjában, amely jelentősen rontja az életminőséget. A betegség korrekt diagnózisa, differenciáldiagnosztikája és a betegek megfelelő kezelése korszerű és bizonyítékokon alapuló kezelési útmutatót igényel. A Magyar Hasnyálmirigy Munkacsoport célul tűzte ki, hogy a jelenleg elérhető nemzetközi irányvonalakat, illetve evidenciákat alapul véve a krónikus pancreatitis kezelésének kulcskérdései vonatkozásában bizonyítékalapú irányelveket fogalmazzon meg. A Magyar Hasnyálmirigy Munkacsoport által kijelölt előkészítő és konzulens munkacsoport lefordította, és ahol szükségesnek találta, kiegészítette és/vagy módosította a nemzetközi irányelveket. Összesen 11 témakörben 123 releváns megállapítás került összeállításra. Az evidencia osztályozása az UpToDate ® rendszere alapján került meghatározásra. Az összeállított irányelvek a 2014. szeptember 12-ei konszenzustalálkozón kerültek bemutatásra és megvitatásra. A résztvevők minden kérdést teljes, illetve erős egyetértéssel fogadtak el. A jelen irányelv hazánkban az első, bizonyítékalapú útmutató a krónikus pancreatitis kezeléséhez. Az irányelv komoly segítséget nyújt a krónikus pancreatitis oktatásához, a mindennapi betegellátáshoz és a megfelelő fi nanszírozás kialakításához. Nesta condição, no entanto, o reparo cirúrgico está associado com recorrência tardia da re-estenose em 10% a 30% dos pacientes. Neste contexto, os avanços tecnológicos na endoscopia terapêutica promoveram a possibilidade alternativa do tratamento efetivo destas obstruções benignas. Considerações em relação ao tempo de reospitalização e de procedimentos devem ser averiguados em detalhes e ponderados em relação á cirurgia. Estenoses malignas estão relacionadas á colangite, icterícia e dor e, consequentemente, com as alterações sistêmicas relacionadas com a sepsis biliar. A conduta endoscópica cria uma derivação do suco biliar para o duodeno, sendo uma verdadeira derivação biliodigestiva endoscópica e utilizando-se próteses plásticas ou metáli-cas. O propósito desta revisão é ofertar aos leitores a eficácia do tratamento endoscópico na estenose benigna e maligna biliopancreática. Em muitos casos a cirurgia oferece o tratamento definitivo com intenção paliativa ou terapêutica, contudo não é isenta de complicações 1 .
