Treatment of Simple Renal Cysts with Single-Session Percutaneous Ethanol Sclerotherapy without Drainage of the Sclerosing Agent

Abstract: Single-session percutaneous sclerotherapy with ethanol is a good option for the treatment of symptomatic renal cysts, as it is highly effective and offers the benefits of a less-invasive procedure.

“…Symptomatic renal cysts can be treated by percutaneous aspiration, with or without the injection of sclerosants, and by open or laparoscopic surgery (9-11). For the symptomatic cases, treatment of the cyst is usually accompanied by remission of the symptoms (12). The decortication of peripheral and peripelvic cysts via laparoscopy is a safe, minimally invasive and effective treatment for symptomatic renal cysts, and this method has a more durable response than that of open surgery (13), yet laproscopy is costly and it must be performed under general anesthesia.…”

“…On the other hand, percutaneous aspiration and sclerotherapy are minimally invasive options. The data that's currently available suggests that ethanol sclerotherapy for treating simple renal cysts has not yet been performed with a standardized method (12). There is no consensus in the scientific literature about the best way to treat simple renal cysts with sclerotherapy (8, 12, 14-16).…”

“…He also exposed specimens of renal cysts to alcohol and discovered that epithelial lining cells were fixed and nonviable within 1-3 min. Most of the studies were carried out with ethanol injection, and it was found to be more effective than only percutaneous drainage in renal cysts [11,[16][17][18]. However, with the leakage of phenol or ethanol into the surrounding tissue, the minor complications such as local tissue corrosion, fever, nausea, pain and microscopic hematuria and the major complications such as aseptic psoas abscesses and severe central nervous system depression are possible [15,16].…”

“…Sclerosing agents are believed to produce local inflammation on the luminal surface of cysts and thus result in the adhesion of cyst the walls [4]. Some sclerosing agents; such as glucose, alcohol, pantopaque, phenol, polidocanol, ethanol, bismuth-phosphate, minocycline have been used in other studies [2,3,4,7,8,9,10,12,19]. However, most of them are ineffective or toxic such as pantopaque and have been abandoned at many centers due to reported complications [11,20].…”
