Treatment of rheumatoid arthritis with an anti‐CD4 monoclonal antibody

Abstract: The effect of treatment with a monoclonal antibody against the CD4 antigen present on T helper cells was studied in 10 patients with severe intractable rheumatoid arthritis. In an open trial, monoclonal antibody 16H5 was infused at a dosage of 0.3 mg/kg of body weight on 7 consecutive days. Studies of the kinetics demonstrated a drastic depletion of CD4+ cells, to as low as 25 cells/pl, 1 hour after the first infusion. The subsequent recovery of the CD4+ cell numbers 24 hours after infusion did not reach initi… Show more

“…Efficacy of antLCD4 monoclonal antibody (MAb) therapy in several murine models, including collagen arthritis (lo), systemic lupus erythematosus (1 l), myasthenia gravis (12), experimental allergic encephalomyelitis (13), diabetes mellitus (14), thyroiditis ( 1 3 , and uveitis (16), strongly supports this view. Pilot studies using several murine MAbs directed against human CD4 in patients with refractory RA have yielded promising results (17)(18)(19)(20)(21). A source of concern in some of these early trials has been the development of host antibody responses directed against the therapeutic M A b (22,23).…”

“…In contrast to the studies with murine CD4 antibodies, the CD4+ T cell counts remained markedly suppressed for up to 12 months after treatment (8)(9)(10)(11). The mechanism of prolonged depletion of circulating CD4+ T cells is not yet understood.…”

“…Transgenic mice were identified by analysis of Pvu II-digested tail DNA. A J,2 probe hybridized with a 5.5-kilobase endogenous band in all mice, but also to a 7.7-kb band in V, 8 mice. All mice were female, homozygous for H-2""' , and were 8 weeks old at the beginning of the experiments.…”
