Radiation therapy (RT) is an important modality in cancer treatment. However, the general perception is that melanoma is radio-resistant. High quality clinical trials are helping to establish the place of RT in select scenarios of advanced disease at primary, regional, and distant sites. New RT techniques need to be integrated with effective new systemic therapies within a multidisciplinary environment to ensure optimum patient outcomes. It is important that radiation oncologists embrace this opportunity.

Malignant melanoma frequently develops cutaneous and/or subcutaneous metastases during the course of the disease. These may present as non-nodal locoregional metastases (microsatellite, satellite, or in-transit) included in stage III or as distant metastases in stage IV. Their presentation is heterogeneous and associated with significant morbidity resulting from both disease-related functional damage and treatment side effects. The standard treatment is surgical excision, whereas local therapies or systemic therapies have a role when surgery is not indicated. Radiotherapy can be used in the local management of ITM, subcutaneous relapses, or distant metastases to provide symptom relief and prolong regional disease control. To increase the local response without increasing toxicity, the addition of hyperthermia and intralesional therapies to radiotherapy appear to be very promising. Boron neutron capture therapy, based on nuclear neutron capture and boron isotope fission reaction, could be an alternative to standard treatments, but its use in clinical practice is still limited. The potential benefit of combining radiotherapy with targeted therapies and immunotherapy has yet to be explored in this lesion setting. This review explores the role of radiotherapy in the treatment of cutaneous and subcutaneous lesions, its impact on outcomes, and its association with other treatment modalities.

IntroductionThe aim of this study was to investigate the dosimetric differences between surface mould high‐dose‐rate (HDR) brachytherapy and external beam volumetric‐modulated arc therapy (VMAT) for two treatment sites.MethodsPreviously treated HDR brachytherapy surface mould scalp (n = 4) and lower leg (n = 3) treatments were retrospectively analysed. The VMAT plans were optimised using an additional 3‐mm setup margin on the clinical target volume (CTV) of the previously treated HDR plans. The HDR plans were calculated and normalised using the TG‐43 formalism and recalculated with Acuros BV (AC).ResultsOn average, the mean brain and normal tissue doses were reduced by 44.8% and 27.4% for scalp and lower leg VMAT cases, respectively, when compared to AC calculated HDR plans. For VMAT plans, the average dose to a 1‐mm thick skin structure deep to the target volume was not any lower than that in AC HDR plans. On average, the CTV coverage was 13.8% and 9.6% lower for scalp cases with AC dose calculation than with TG‐43 and 8.3% and 5.3% lower for lower leg cases if 0‐ or 1‐cm backscatter material was applied above the catheters, respectively.Conclusions VMAT is a feasible treatment option in the case of extensive skin malignancies of the scalp and lower leg. Uncertainties related to delivered dose with HDR brachytherapy when using the TG‐43 dose calculation model or possible air gaps between the mould and skin favour the use of VMAT. The potential soft tissue deformation needs to be considered if VMAT is used.

Abstract: There was a CR in all cases without in-field recurrence. To our knowledge, this is the first reported experience in treating skin melanoma with a BT surface mould. We recommend that BT surface mould should be considered when treating patients with RCNM.
