“…For example, outbreaks of nosocomial infections have been traced to contaminated prosthetic heart valves 156 and intraocular lenses. 157,158 Similarly, failure to adhere to guidelines for high-level disinfection of semicritical items, especially endoscopes, has led to multiple nosocomial outbreaks. More than 10 outbreaks and 30 pseudo-outbreaks have been related to inadequately disinfected bronchoscopes, 159 and more than 30 outbreaks have been associated with gastrointestinal endoscopes.…”

“…The main factors contributing to post-operative ocular infection are still not fully recognized, though the step which involves IOL implantation logically plays a role in the process of contamination as it has been shown that microorganisms can adhere to the surface of the IOL [10]. This is specially important in view of the fact that often the lenses are placed in contact with the cornea or conjunctiva just before their implantation [9,19]. The IOL is the only item that goes into the eye during surgery, but does not come out.…”

“…There have been numerous reports of bacterial colonisation of intraocular lenses in both experimental"' 19 and clinical"'"24 settings, but the uncertainty as to whether or not to remove the lens continues.25 Successful treatment of different forms of endophthalmitis has been reported with the prosthesis left in situ,2 12 Zambrano et al I have suggested that the severity of the condition should dictate the therapeutic strategy. All six of our cases fall into the more florid category and justify our radical approach.…”
