Treatment of Photoaged Skin with Topical Tretinoin

Abstract: Once-daily application of tretinoin to photodamaged facial skin for 0-12 months results in substantial clinical and histologic improvement. As regards appearance, fine wrinkles become effaced, dyspigmentations fade, surface becomes smooth and develops a ‘rosy glow’. Histologically, atrophy and dysplasia of the epidermis are completely corrected. New collagen is laid down subepidermally along with the development of new small vessels (angiogenesis). Mesenchymal dermal cells become more numerous and larger. The … Show more

“…Studies of the effect of tretinoin treatment on the dermis of hairless mice [27,28] have found evidence for angiogenesis, while con flicting evidence has been reported in studies involving human subjects with photodamaged skin [10,20,[29][30][31]. The transient in crease in HbCb concentration with tretinoin treatment documented in this study does not support a claim for angiogenesis.…”

“…Thus, in histologic studies, following the topical application of 0.05% tretinoin, one can observe hyperplasia of the epidermis with elimination of dysplasia and atypia, melanosome dispersion, and the induction of dermal collagen formation 30,36 . The use of relatively high concentrations of GA, citric acid, or lactic acid (25%) over 6 months (twice daily application) can also induce hyperplasia, with decreased atypia, improved dispersion of melanosomes, and thicker elastin fibers 14 .…”

“…114,115 Topical retinoids represent one medical therapy capable of reorganizing dermal collagen and blood vessels, while also providing an anti-inflammatory effect. 42,43,46,[116][117][118][119][120] While it is clear that studies are required to establish the efficacy of this approach for rosacea, one author (M.T.P.) has been utilizing a topical retinoid approach adjunctively for rosacea for 5 years.…”
