Treatment of Mandibular Non-union Using Patient Specific Crib Cage Plates and Cellular Bone Allograft: A Case Report

Abstract: A gunshot wound to the mandible frequently creates a comminuted fracture that can be debilitating for the patient and challenging for the surgeon. In some instances, immediate open reduction and rigid fixation is not possible, and closed reduction with intermaxillary fixation is employed. This may lead to non-union or mal-union of the segments. This case report describes the management of mal-union of bilateral comminuted mandibular angle fractures secondary to a gunshot wound injury. The mandibular fractures … Show more

“…By providing all 3 essential bone remodeling properties, VF-CBA facilitated fusion at the defect site. Although VF-CBA has been used successfully to achieve fusion in spine and orthopedic applications, [13][14][15][16][17][18] its use in oral and maxillofacial surgery is an emerging application, [19][20][21] and in particular, to our knowledge, has not yet been shown in a pediatric patient. Therefore, this is the first known report of a successful immediate reconstruction of a large segmental mandibular defect using VF-CBA in a young child.…”

“…While not readily generalizable, the results from this case support the continued research for VF-CBA use in oral and maxillofacial surgery, consistent with previously published results. [19][20][21]…”

“…Vivigen (DePuy Synthes, Warsaw, IN), a cellular bone allograft, has been used in conjunction with patient-specific crib/cage plates to treat nonunion in gunshot wound trauma. 56 DBM has also been used successfully to fill skull base defects. 57 Demineralized perforated block bone, which is processed similarly to DBM but kept intact without morselization, has been utilized for cranial reconstruction safely without concern for immunogenic reaction.…”
