Treatment of malignant leydig cell tumor

Abstract: Malignant Leydig cell tumors (LCT) are rare. Only 32 cases of malignant LCT (as evidenced by metastatic spread) were reported. Generally metastatic spread occurs within 2 years of the primary LCT, and the patient dies within 2 years of the discovery of metastatic disease. The tumor is highly resistant to both radiation and chemotherapy. It also has a great propensity for recurring after surgical resection. A case is reported of a patient whose metastatic disease occurred 8 years after his primary LCT had been … Show more

“…4 It is very important to identify such cases as many cases have been reported which had deranged endocrine levels and appearance of atypical symptoms even after years of unilateral orchidectomy. 5 Our case also presents with normal hormonal levels, henceforth we must identify benign LCTs and do long term follow up.…”

“…Long-term excess of estrogen can cause impairment of spermatogenesis, most likely because of hypothalamic-pituitary inhibition as well as direct blockade at the testicular level. 16 This calls for early detection and management of these tumors to preserve the reproductive capacity.…”

“…Commonly, the surgical management of LCTs is the radical orchiectomy (10,11). Nevertheless, an organ sparing surgery was reported by Suardi in small LCTs with benign characteristics (12, 13).…”

“…If the tumor is malignant, orchiectomy with retroperitoneal lymph node dissection is recommended, since chemotherapy and radiation therapy show limited efficacy in the treatment of malignant Leydig cell tumors. Survival time of patients with this malignancy range from 2 months to 17 years, and the mean survival time is 2 years (10,11). To improve the survival time of patients with malignant Leydig cell tumor, and to decrease the serious consequences, such as testicular dysfunction and infertility that is caused by the current treatment for benign Leydig cell tumors, alternative treatments need to be explored.…”
