Treatment of lymphocele in renal transplant recipients by laparoscopic fenestration after transcutaneous staining

Abstract: Lymphocele is a rare cause of impairment of graft function after renal transplantation. Treatment ranges from external drainage to laparoscopic internal marsupialization. A therapeutic approach to the treatment of symptomatic lymphocele is described, which employs external drainage, if necessary followed by methylene blue staining and laparoscopic internal drainage, should conservative treatment fail. Five patients developed post-transplant lymphocele of varying size and were treated according to this protocol… Show more

“…These include methylene blue instillation to distend and mark the cavity [21] and preoperative wire localization with ultrasound or CT guidance [25]. The successful use of intraoperative transcutaneous or laparoscopic ultrasound has also been described, although the ability to identify the ureter varies [19].…”

“…Laparoscopic peritoneal drainage has slowly moved into the focus of attention during recent years [15,16,21,22] . This technique requires greater operative skill and supportive facilities than percutaneous approaches.…”

“…According to positive results in the treatment of posttransplant lymphoceles [15,16] , we established a simplifi ed laparoscopic transperitoneal marsupialization of iliac lymphoceles after pelvic lymphadenectomy in patients with prostate cancer using methylene blue instillation.…”

“…The laparoscopic approach offers the advantages of minimal trauma and fast recovery, as concluded by almost all authors [l, 5,8,10,13,19,211. However, according to several reports [lo, 13,17,201, and demonstrated by our early experience, there is a significant risk of injury to vital structures (ureter/renal pelvis/ bladder/iliac vessels).…”
