Periorbital dark circles (PODC) are a common worldwide cosmetic problem. It is difficult to treat due to complications in its pathogenesis and aetiology. Available lines of treatment for PODC include whitening creams, topical retinoid acid, chemical peeling, laser therapy, carboxytherapy, autologous fat transplantation, injectable fillers and surgery (blepharoplasty).The aim of this study isto evaluate and compare the efficacy of chemical peels using trichloroacetic acid (3.75%) and lactic acid (15%) in a gel formula with that of carboxytherapy, in the treatment of periorbital hyperpigmentation.Two groups of patients with PODC were included in the study, named Group A and B in which each group consisted of 15 patients. Group A was assigned for patients who received treatment with chemical peeling with a combination of trichloroacetic acid (3.75%) and lactic acid (15%) in a gel formula, once a week for four weeks. Group B was assigned for patients who received carboxytherapy that was performed by subcutaneous and intradermal injection of CO 2 once a week for four weeks. All patients were assessed by digital photographs, before and after treatment, by observing the improvement in the grade of PODC. Reports of patient satisfaction and global tolerance were evaluated by three medical observers. There was a significant improvement in the grade of PODC in both groups. The degree of improvement of PODC in group A was excellent, with good grade in 93.4% of the treated patients while fair grade in 6.6% of them. There was a statistically significant improvement in the pigmented type. The degree of improvement of PODC in group B was excellent, with good grade in 86.7% of the treated patients while fair grade in 13.3% of them. However, no statistically significant difference between the two groups was observed. Minimal and transient side effects were noticed; however, it did not require further treatment. In conclusion, the two methods of treatment were effective in the treatment of PODC, with the improvement of PODC observed from the first treatment session with both chemical peeling and carboxytherapy.

L Métodos objetivos para análise de estudos em dermatologia cosméticaObjective methods for analyzing outcomes in research studies on cosmetic dermatologyResumo: A Dermatologia Cosmética é uma área em constante crescimento, motivo pelo qual se faz necessária a utilização de métodos objetivos para validar os resultados dos estudos científicos. As técnicas mais empregadas na maioria desses trabalhos são: histopatologia, imunoistoquímica, morfometria, estereologia, fotografia digital, biometria, profilometria óptica e microscopia confocal. Esta revisão tem como objetivo trazer informações sobre os principais métodos utilizados como ferramenta de análise dos resultados obtidos, além de auxiliar o dermatologista a aguçar seu senso crítico em relação às publicações e apresentações que se utilizam de métodos subjetivos de avaliação. Palavras-chave: Dermatologia; Eficácia; Métodos Abstract: Cosmetic dermatology is a field of medicine that is in constant development; therefore, the use of objective methods for validating the findings of scientific studies is crucial. The most commonly used techniques in the majority of these studies include histopathology, immunohistochemistry, morphometry, stereology, digital photography, biometry, optical profilometry and confocal microscopy. The objective of this review was to provide an update on the principal methods used as tools for analyzing outcomes and also to provide the dermatologist with means of sharpening his/her critical judgement with respect to the publications and presentations that use subjective evaluation methods.

RESUMOA região periorbital é uma das primeiras áreas a mostrar sinais do envelhecimento, como rugas, flacidez e hiperpigmentação periorbital (HPO). A HPO interfere na aparência facial provocando aspecto de cansaço, tristeza ou ressaca. Possui etiopatogenia complexa que inclui múltiplos fatores que podem ser de causa primária ou secundária. Os fatores genéticos são considerados de causa primária, enquanto os ambientais são de causa secundária. Assim, são exemplos de fatores ambientais o excesso de exposição solar, a hiperpigmentação pós-inflamatória, o excesso de vascularização subcutânea, a hipertransparência da pele, o edema periorbital e a herniação da gordura palpebral. A gravidade clínica da doença pode variar e geralmente a hiperpigmentação está presente bilateralmente e simetricamente ao redor dos olhos. O propósito deste estudo foi realizar uma revisão integrativa com o objetivo de discutir e avaliar as causas e fatores etiológicos e descrever os possíveis tratamentos disponíveis na literatura. Foram utilizados 26 artigos que abordavam os temas: anatomia, etiologia, causas e tratamentos. Tendo em vista a escassez de informações sobre a etiologia, bem como um tratamento consensual, é necessário um exame clínico adequado para direcionar a conduta. Existem várias sugestões para tratamento da HPO descritos, como peeling, preenchimento com gordura autóloga, laser, utilização de cosméticos, injeções de plasma rico em plaquetas e carboxiterapia. Palavras-chave: envelhecimento da pele; pálpebras; hiperpigmentação; edema; técnicas cosméticas. ABSTRACTThe periorbital region is one of the first areas to demonstrate signs of aging such as wrinkling, skin laxity and periorbital hyperpigmentation (HPO). The HPO interferes in the facial appearance resulting on a tired, sadness or hangover aspects. It has a complex etiopathogenesis with factors that could be by primary or secondary cause. The genetic factors are stated as a primary cause and the environmental factors are stated as a secondary cause. In that way, there are many examples of environmental factors such as excess of sun exposition, post inflammatory hyper pigmentation, excess of subcutaneous vascularization, hyper transparency of the skin, periorbital edema, and fat herniation eyelid. The clinical severity of the condition varies and normally the hyper pigmentation is present bilaterally and symmetrically around the eye region. The main objective of this study was to develop and evaluate the causes and etiological factors, and to describe the possible treatments available in the current literature. It was utilized 26 scientific papers on the following subjects: anatomy, etiology, causes and treatments. Owing to the information scarcity about its etiology, as well as its consensual treatment, it is necessary an adequate clinical examination in order to provide directions to the conduct. There are several suggestions for HPO treatment described: peeling, fillers, autologous fat fillers, cosmetic use, injection of platelet rich plasma (PRP), and carboxytherapy. ...

Treatment of Idiopathic Cutaneous Hyperchromia of the Orbital Region (ICHOR) with Intense Pulsed Light

Abstract: HEPL was shown to be useful in clearing up ICHOR. This was maintained after 1 year. Epidermal and dermal histopathology showed decreased melanin following treatment. Longer follow-up is needed to evaluate possible later recurrence of ICHOR.
