Objective: To compare the effects of a unique fixed combination levothyroxine/liothyronine (LT4/LT3) therapy in patients with primary hypothyroidism. Subjects and methods: This is a randomized, double-blind, crossover study. Adults with primary hypothyroidism (n = 32, age 42.6 ± 13.3, 30 females) on stable doses of LT4 for ≥ 6 months (125 or 150 μg/day) were randomized to continue LT4 treatment (G1) or to start LT4/LT3 therapy (75/15 μg/day; G2). After 8 weeks, participants switched treatments for 8 more weeks. Thyroid function, lipid profile, plasma glucose, body weight, electrocardiogram, vital signs, and quality of life (QoL) were evaluated at weeks 0, 8 and 16. Results: Free T4 levels were significantly lower while on LT4/LT3 (G1: 1.07 ± 0.29 vs. 1.65 ± 0.46; G2: 0.97 ± 0.26 vs. 1.63 ± 0.43 ng/dL; P < 0.001). TSH and T3 levels were not affected by type of therapy. More patients on LT4/ LT3 had T3 levels above the upper limit (15% vs. 3%). The combination therapy led to an increase in heart rate, with no significant changes in electrocardiogram or arterial blood pressure. Lipid profile, body weight and QoL remained unchanged. Conclusions: The combination therapy yielded significantly lower free T4 levels, with no changes in TSH or T3 levels. More patients on LT4/T3 had elevated T3 levels, with no significant alterations in the evaluated outcomes. No clear clinical benefit of the studied formulation could be observed. Future trials need to evaluate different formulations and the impact of the combined therapy in select populations with genetic polymorphisms. Arch Endocrinol Metab. 2016;60(6):562-72

ÖZAmaç: Hipotiroidi tüm yaş gruplarında görülebilen, bireylerin sağlıklarının genel olarak bozulmasına neden olan klinik bir tablodur. Tedavisi genellikle yaşam boyu ilaç kullanımıdır. Bu tedaviye rağmen hormon yetmezliğine bağlı semptomlar görülmekte ve uzun dönem ilaç kullanımına bağlı yan etkiler ortaya çıkmaktadır. Hastalardaki yetersiz tiroid hormon miktarını optimum sınırlara çekmek ve hastaların yaşam kalitesini yükseltmek için tiroid dokusu transplantasyonu alternatif bir yöntem olarak önem taşımaktadır. Sunulan çalışmada, transplantasyonda immün redden etkilenmemek için hem doku kültürü hem de biyouyumlu polimerle kapsüle edilmiş tiroit dokusunun in vitro optimizasyonu amaçlanmıştır. Gereç ve Yöntemler:Mekanik yollarla kültüre edilen tiroid dokusu 2 grupta takip edilmiştir; doku kültürü ve kapsülasyon kültürü. In vitro ortamda 60 gün boyunca takip edilen kapsüllerin morfolojileri ışık mikroskopu ile birlikte T3 ve T4 hormon değerleri ise biyokimyasal olarak değerlendirilmiştir.Bulgular: 3., 9., 12., 15., 30., 45., 60. günlerde toplanan medyum örneklerinden T3 ve T4 seviyeleri gözlemlendi. Kapsüle edilen dokularda, kapsüle edilmeyenlere ve negatif kontrol dokularına göre 15. gün itibariyle T3 değerleri daha yüksek bulundu. Buna rağmen T4 seviyeleri 60 gün boyunca sabit kaldı.Sonuç: Hipotiroidinin kalıcı tedavisi için kapsüle edilen tiroid dokularının in vitro ortamda verimliliği incelenmiştir. Öncül sonuçlarımız umut vaat etmesine rağmen bu optimizasyonun in vivo hayvan çalışmalarında da doğrulandıktan sonra insan çalışmalarına geçilmesi düşünülmelidir.Anahtar Sözcükler: Hipotiroidi, Aljinat tabanlı kapsülasyon, Tiroid hiperplazi, Hürthle hücreli karsinom ABSTRACT Objective: Hypothyroidism is a clinical condition that affects all age groups. Generally, lifelong medication is required for the treatment. Despite treatment, symptoms that are related to hormone insufficiency and side effects due to long-term medication can be seen. As an alternative approach, thyroid tissue transplantation may be useful, because of its potential to elevate the thyroid hormone levels and thus increase daily quality of life for the patients. Our aim in this study was to detect the in vitro optimal microencapsulation conditions for thyroid tissues with a biocompatible polymer to avoid immune rejection following transplantation. Material and Methods:Thyroid tissues were isolated mechanically, and followed-up in two groups; tissue and capsulated tissue culture. In-vitro capsule formation and morphology were screened using brightfield microscopy and T3 and T4 levels were evaluated biochemically throughout 60 days.Results: T3 and T4 levels were screened from collected medium samples at day 3., 9., 12., 15., 30., 45., 60.. In capsuled tissues T3 level was detected higher than the non-capsuled and negative control tissues from day 15. However, T4 level was stable for 60 days.
