GirişGlenohumeral eklem dejenerasyonun seyri ve nedenleri vücuttaki diğer diartrodiyal eklemlerde olan osteoartrit ile benzerdir. Ancak glenohumeral eklem yük taşıyan bir eklem olmadığı için diz ve kalça eklemlerine kıyasla primer osteoartrit daha nadir gö-rülmektedir, diz ve kalça eklemlerinden sonra dejeneratif hastalıklardan üçüncü sık-lıkta etkilenen büyük eklemdir. [1,2] Epidemiyoloji, Etyoloji ve Patoanatomi Omuz eklemi diz ekleminden sonra en çok kronik ağrı rapor edilen ikinci eklem olmasına rağmen (sırayla %30.6 ve %63.4), klinik olarak önemli derecede artrit nadir olarak görülmektedir.[3] Omuz artroskopisi uygulanan hastalarda %4 ile %17 arasında kartilaj hasarı görülmekle beraber, gerçek sıklığında belirsizlikler vardır ve bu asemptomatik kartilaj hasarının prognozu hakkında yeterli bilgi bulunmamaktadır. Genel olarak omuz ağrısı ile değerlendirilen hastalarda %5-10 arasında dejeneratif glenohumeral artrit tespit edilmektedir. İnsidans bayanlarda yüksektir ve 60-70 yaş aralı-ğında artmaktadır. [4,5] Glenohumeral artritin etyolojisi primer ve sekonder nedenler olarak iki gruba ayrıl-maktadır, ancak primer artritte birtakım çevresel ve genetik faktörler rol alsa da etkenler çok faktörlü olabilir ve artritin nedenini bulmak zor olabilmektedir. Primer glenohumeral artritin en sık etyolojisinde rotator manşet yırtığı artropatisi, travma sonrası artrit ve romatoid artrit yer alırken, postoperatif artrit, kondrolizis ve osteonekrozis sekonder osteoartrit nedenleri arasında sayılmaktadır. [6,7] Humerus başı merkezindeki kıkırdak periferinden daha kalındır ve perifere gidildikçe kıkırdak kalınlığı azalır, bunun tam tersi yönünde glenoid merkezindeki kıkırdak periferinden daha incedir ve bu durum glenohumeral eklem uyumunu sağlar. ABSTRACT:The presence of either a Hill-Sachs or a bony Bankart defect has been indicated as a possible cause of subluxation and anterior shoulder dislocation. Previous studies investigated only the effects of isolated humeral or glenoid defects on glenohumeral instability. We investigated the effects on shoulder stability of both glenoid and humeral defects in the glenohumeral joint. A computerbased finite element approach was used to model the joint. A generic model was developed for cartilage and bone of the glenoid and humerus, using previously published data, and experiments were analyzed using static analysis with displacement control in the anterior-inferior direction. Simulations were run with a 50-N compressive load in the presence of both isolated and combined defects to analyze reaction forces and distance to dislocation. The distance to dislocation for normal joint was 13.6 mm at 908 abduction, which reduced to 9.7, 0, and 0 mm for largest isolated humerus defect, glenoid defect, and certain combined defects, respectively. For combined defects, stability ratio was decreased to 0% from 43%. Our results suggest that in the setting of combined bone defects, stability may be reduced more than what is known for isolated defects alone.
