: Distal femoral fractures are much less common than hip fractures and account for 7 % of all femoral fractures. To determine the fracture characteristics (AO type, Muller’s classification). To study the advantages of fixation with distal femoral locking compression plates and retrograde intramedullary supracondylar nailing. The present comparative study was conducted between May 2019 to November 2020 in department of othropeadics. Sample size of 30 patients presenting with supracondylar fractures of the femur were treated by distal femoral locking compression plates, dynamic condylar screws and retrograde supra condylar nailing.Out of 30 cases those treated by supracondylar nail had an average healing time of 14.8 weeks and those treated by locking compression plate had an average healing time of 15.9. Among the 30 cases of supracondylar fracture fixed by supracondylar nail 14 were scored as good and one was scored as excellent, whereas those fixed by locking compression plate 14 were scored as good and 1 cases was scored as fair. Out of 30 cases of supracondylar fracture fixed none had any intraoperative complications barring one which had break of drill bit intraoperatively and stayed insitu to the bone, there were no intances of surgical wound infections. The intraoperative blood loss was significantly lower when the fractures were fixed by supracondylar nail which averaged to 150ml when compared to the fixations by locking compression plate which averages to 300ml.The supracondylar nail is more ideal when compared to the distal femoral locking compression plate as there is less operation time, less blood loss, more range of movement, less soft tissue stripping & faster radiological union when compared to the distal femoral locking compression plate. Background: Distal femoral fractures reportedly account for less than 1% of all fractures and comprise between 4%-6% of all femoral fractures. The current method of open reduction and internal fixation has become evident in the recent years with good results being obtained with the AO blade plate, dynamic condylar screw and other implant systems like intramedullary retrograde nails and recently locking compression plate. Controversy still remains regarding the optimum device for distal femur fixation. Objectives: To compare functional outcomes of fractures of distal femur operated with distal femur locking compression plate and distal femur nail using range of motion at knee and Neers knee scoring system and to compare rate of complications associated with both treatment modalities. Materials and methods: In this retrospective study, 60 patients with displaced distal femur fractures presented to our hospital were studied from January 2012-june 2017. 30 patients underwent fixation with locking compression plate and rest 30 underwent distal femur nailing. Outcomes were measured and assessed at 1 months, 3 months and 6 months using Neers knee scoring system. Results: Union was achieved in all patients with mean time to radiological union being 20 weeks in retrograde nailing patients and 24 weeks in locking compression plating. Neer scores were graded as excellent in 51.7 % in retrograde nailing patients and 56.6 % in plating patients. Mean range of motion was more (115 degree) in retrograde nailing patients compared to locking compression plating patients (114 degree) at the end of follow up. Conclusion: There were no statistically significant differences found in functional outcome of the patients between the two surgical methods. Locking compression plate is better implant in comminuted intra-articular fractures of distal end of femur, particularly in elderly patients with osteoporotic bone. Retrograde intramedullary nailing is a good operative technique for extra-articular distal third femoral fractures. However, both techniques require sufficient surgical experience and appropriate preoperative planning.

Efficacy of dynamic condylar screw for the treatment of distal femoral fractures: a prospective study

The incidences of distal femoral fractures are more common in males around the age of 30 and in elderly females. Dynamic Condylar Screw is cost-effective and procedure relatively easy to perform and affords a rigid internal fixation. Materials and Methods: This prospective study was done on 56 patients aged above 18 years with distal femur fractures. The patients were operated under spinal anaesthesia. A sand bag was used under the ipsilateral hemi pelvis. Suction drain was used in all cases. Radiographs were taken on 1st POD. Change of dressing and drain removal was done on 2nd POD. Suture removal was done on 10th POD. Follow-up radiographs were taken at 6 weeks, 3 months and 6 months. Results: The predominant age group for the femoral fractures is 31-40 years (35.7%) followed by 41-50 years 32.1%). Type A fractures were seen in 76.78% of the cases, type B in 5(8.92%) and type C in 8(14.28%) of patients. Overall results were assessed using Knee Society Score. 31(55.36%) patients had excellent results, 14(25%) had good, 9(16.07%) had fair and 2(3.57%) had poor outcomes. Conclusion: DCS remains a good implant for distal femur fractures particularly types A and B. It is easier to perform, less difficult technically and is also very satisfying to the surgeon and patients as the outcomes are very good.

Abstract: The trends towards the benefits of the LISS procedure when correctly applied would suggest that not only should the LISS procedure be performed for distal femur fractures, but the correct principle of insertion is important in improving the patient's outcome.
