Purpose To evaluate the effect of scar overlapping suture for treating chronic tendinous mallet finger deformity in children. Methods Six patients younger than 18 years were investigated retrospectively. The active extensor lags of the distal interphalangeal joint (DIPJ) were all more than 40°, and the passive ranges of DIPJ motion were normal. They were all treated surgically by scar overlapping suture technique, featuring careful overlapping suture of the extensor scar and temporary transarticular Kirschner wire fixation of the DIPJ. Results Average follow-up was 3.1 years (ranging from 2 to 5 years). All patients made significant improvement in DIPJ activity. Three patients achieved full active DIPJ extension, whereas one patient had a 10° extensor lag and two patients had 5° extensor lags. All patients achieved normal active flexion ranges and full passive motion ranges of DIPJ compared with their uninjured side. There was no bone dysplasia, pain, or deformity recurrence. Conclusions Scar overlapping suture for treating chronic tendinous mallet finger in children is safe and effective. According to the Crawford criteria, all patients were graded as excellent.

Outcome of Non-Surgical Treatment of Mallet Finger

Objective: To establish the association between initial and residual angulation of the distal interphalangeal joint (DIJ) in mallet finger treated conservatively. Methods: An observational, prospective, descriptive and analytical research developed with uncomplicated closed mallet finger patients between January and December 2017. A total of two measurements of the DIJ were done, at the initial trauma and 6 weeks after conservative treatment. All measurements were ranked according to the Crawford Classification and Relative Risk was measured. Results: In total, 43 patients were studied, in which 53.48% of outcomes obtained were excellent. The sample was divided in two groups; one with less than 30º of DIJ initial angulation, which had 28% of residual angulation. The second group with more than 30º presented 72.22% of residual angulation. The Relative Risk to present a residual angulation in patients that had 30º of DIJ initial angulation was 2.99 (CI 95%) with p = 0.0059. Conclusion: It is suggested that patients with an initial DIJ angulation more than 30º are more likely to present residual angulation with conservative treatment. Level of Evidence IV, Case series.

Çekiç parmak deformitesi DIP eklemde aktif ekstansiyon kaybı ve ilerleyen süreçte PIP eklemde hiperekstansiyon deformitesi olarak görülür. Çekiç parmak deformitesinde çeşitli tedavi seçenekleri bulunmasına rağmen optimal tedavisi halen belirsizdir. Bu yazıda künt yaralanma sonrası geç dönemde çekiç parmak deformitesi gelişen 7 yaşında olguda sekonder cerrahi ve sonrasında fizyoterapi programının sonucu sunuldu. Çekiç parmak deformitesi nedeni ile ekstansör tendon plikasyonu yapılan olguda atelleme, fizyoterapi ve düzenli takip başarı ile sonuçlandı.
