Necrotizing fasciitis is a severe soft tissue infection that results in necrosis of the fasciae and subcutaneous tissues; the infection can quickly prove fatal. Although involvement of the head and neck is rare, causes are usually odontogenic or pharyngeal but can also be insect bites, local trauma, burns or surgery. We present a clinical case of a 31-year-old Italian woman with cervical necrotizing fasciitis having an uncommon presentation. While under treatment, the patient's husband was admitted for necrotizing fasciitis of the medial fasciae of his left leg subsequent to an insect sting. The causes, diagnosis and treatment of necrotizing fasciitis are reviewed.

ÖZServikal nekrotizan fasiit, daha çok yaşlılarda ve immün yetmezliği bulunan hastalarda görülen, polimikrobiyal kökenli, hızlı ve yıkıcı seyreden ve bazen ölümcül olabilen bir derin boyun enfeksiyonudur. Nadiren enfeksiyon derin fasiyal planlar aracılığı ile mediastene yayılır ve desendan nekrotizan mediastenite neden olur. Desendan nekrotizan mediastinit yüksek mortaliteye sahip, acilen tanı konulup tedavi edilmesi gereken ciddi bir tablodur. Bu derlemede biri tip 1, diğeri de tip 2B desendan nekrotizan mediastinit tanısı ile servikal ve mediastinal drenaj ile tedavi edilmiş iki olgu sunuldu ve konu ile ilgili güncel literatür tartışıldı.Anahtar Sözcükler: Servikal nekrotizan fasiit; desendan nekrotizan mediastinit; mediastinoskopi; torakotomi. ABSTRACTCervical necrotizing fasciitis is a polymicrobial, rapidly progressive, destructive, and sometimes fatal deep neck infection, that is most commonly seen in the elderly and immunocompromised patients. The infection rarely spreads to mediastina via deep fascial planes and causes descending necrotizing mediastinitis. Descending necrotizing mediastinitis is a serious clinical entity with a high mortality rate that should be promptly diagnosed and treated. In this review, we present two cases with type 1 and type 2B descending necrotizing mediastinitis treated with servical and mediastinal drainage, and also discuss relevant contemporary literature.

Nekrotizan fasiit önemli derecede morbidite ve mortalite ile ilişkili deri ve fasyanın ciddi bir enfeksiyonudur. Nekrotizan fasiit, baş ve boyunda tutulumu nadir olarak gözlenir. Servikal nekrotizan fasiitin standart tedavisi, intravenöz antibiyotik ve acil olarak cerrahi debridman uygulamayı içerir. Hiperbarik oksijen terapisi, nekrotizan fasiitin kontrolünde adjuvant olarak kullanılmaktadır. Birçok çalışmada hiperbarik oksijen kullanıldığında daha kısa hastanede kalma süresinin olduğu, mortalitenin ve cerrahi debridman sayısınının azaldığı ileri sürülmektedir. Bu çalışmada servikal nekrotizan fasiit tanısı konmuş olup boyunda geniş bir doku defekti oluşan ve intravenöz antibiyotik, cerrahi debridman yanında adjuvant olarak hiperbarik oksijen terapisi uygulanan bir hasta sunulmuş ve ilgili literatür gözden geçirilmiştir.Anahtar Kelimeler: Fasiit, hiperbarik, servikal, nekrotizan, oksijen Necrotizing fasciitis is a serious infection of the fascia and skin associated with considerable morbidity and mortality. Necrotizing fasciitis rarely involves the head and neck. The standard treatment for cervical necrotizing fasciitis includes intravenous antibiotics and prompt surgical debridement. Hyperbaric oxygen therapy is used as adjunctive therapy in managing necrotizing fasciitis. Several reports suggest shorter hospital duration, reduced mortality, and decreased surgical debridement when hyperbaric oxygen is used. In this report, a patient who was diagnosed with cervical necrotizing fasciitis having a large tissue defect on her neck and was administered intravenous antibiotics, surgical debridement, and adjuvant hyperbaric oxygen therapy is presented, and the related literature has been looked through.Key Words: Fasciitis, hyperbaric, cervical, necrotizing, oxygen GirişNekrotizan fasiit (NF) cildi ve cilt altı dokusunu tutan hızlı bir şekilde ilerleyen hayatı tehdit eden fasyanın bakteriyel enfeksiyonudur (1). NF gelişen hastaların çoğunda diabetes mellitus (DM), ileri yaş, akut veya kronik renal yetmezlik, doğum sonrası dönem, alkolizm, intravenöz ilaç kullanımı, malnutrisyon, malignite, periferal damar hastalığı, radyasyona maruziyet gibi immün sistemi zayıflatan durumlar mevcuttur (2, 3).Nekrotizan fasiit baş ve boyun bölgesinde nadiren görülür (4, 5). Servikal nekrotizan fasiit (SNF) derin boyun infeksiyonlarının agresif ve potansiyel ölümcül bir formudur. SNF'in mortalite oranı yüzde 40 ila 76 arasında değişmekte olup yüksek bir oran göstermektedir (5, 6). Bu durum erken klinik bulguların olmaması, hastalığın hızlı bir progresyon göstermesi ve cerrahi müdahalenin gecikmesine bağlanmaktadır (4, 7). Fasya yüzeyleri tarafından sınırlanmış tipik derin boyun enfeksiyonundan farklı olarak SNF'e yol açan organizmalar hızlı bir şekilde bu yüzeylere yayılır ve diğer dokuları da etkiler (7). Sonuç olarak derin dokuların ve cildin yaygın destrüksiyonu meydana gelir (1, 5).Servikal nekrotizan fasiit önemli morbidite ve mortalite nedenidir.
