Treatment of AIDS-Related Cutaneous Kaposi's Sarcoma With Topical Alitretinoin (9-cis-Retinoic Acid) Gel

Abstract: The results show that alitretinoin gel application is safe and generally well tolerated, and they indicate the superiority of alitretinoin 0.1% gel over vehicle gel in the treatment of cutaneous AIDS-related KS lesions.

“…AIDS-KS is known to be responsive to HAART, and control of HIV load is the primary goal in the treatment of AIDS-KS [46]. Pain, edema and cosmetic concerns arising even from slowly progressive disease can be addressed with local therapies, such as irradiation [47,48] or excision, alitretinoin gel (a retinoid approved for topical application) [49], IFN-α [50], or experimental therapies [51], such as novel angiogenesis inhibitors (reviewed in [46,[52][53][54]). Advanced or quickly progressing disease can be treated with systemic cytotoxic chemotherapy.…”

“…Topical 9-cis-retinoic acid (alitretinoin, Panretin ® , Ligand Pharmaceuticals Inc.) gel has recently been licensed for use in patients with cutaneous KS and has been associated with a response rate of up to 50% [77]. Intralesional chemotherapy with vincristine (Oncovin ® , Clonmel Healthcare Ltd) or vinblastine (Velbe ® , Clonmel Healthcare Ltd) gives complete or partial response rates of 60 to 90% in cutaneous lesions, while a second series of injections is often necessary [26,78].…”
