Treatment of acute high-grade acromioclavicular joint dislocation

Abstract: Acromioclavicular (AC) joint dislocations account for about 9% of shoulder injuries. Among them, acute high-grade injury following high-energy trauma accounts for a large proportion of patients requiring surgical treatment. However, there is no gold standard procedure for operative treatment of acute high-grade AC joint injury, and several different procedures have been used for this purpose in clinical practice. This review article summarizes the most recent and relevant surgical options for acute high-grade … Show more

“…Horizontal instability (AC ligament disruption) has received less attention [1,9,10]. There is currently no gold standard surgical treatment for any type of AC injury, especially for horizontal ACJ instability [4,8,10,11]. This study examined the effects of two Ethibond suture techniques, the loop technique and the two holes in the clavicle technique, on the horizontal stability of the ACJ following its complete dislocation.…”

“…Various surgical options are available for the treatment of acute AC joint dislocations. These include bandaging, fixation of the AC joint with pins, tension band wiring using the modified Weaver-Dunn procedure, fixation with washer and screw, and clavicular plate [ 5 , 7 , 13 , 27 ]. Among these, the hook plate fixation technique shows excellent security.…”

“…3,5 El tratamiento de una luxación AC depende de su grado de lesión de acuerdo con la clasificación de Rockwood y la demanda funcional del paciente. 6 Los mecanismos más comunes de lesión de la articulación AC incluyen caída sobre un brazo en extensión o una contusión directa con carga axial en la cara lateral del hombro con el brazo en aducción. 7 Los pacientes refieren dolor en la parte superior del hombro durante intentos de elevación de la extremidad superior; presentan dolor a la palpación de la articulación AC.…”
