Treatment Failure after Multiple Courses of Triclabendazole among Patients with Fascioliasis in Cusco, Peru: A Case Series

Abstract: Triclabendazole is reported to be highly effective in treatment of human fascioliasis. We present 7 of 19 selected cases of human fascioliasis referred to our center in the Cusco region of Peru that failed to respond to triclabendazole. These were mostly symptomatic adults of both sexes that continued passing Fasciola eggs in the stool despite multiple treatments with 2 doses of triclabendazole at 10 mg/kg per dose. We documented the presence of eggs by rapid sedimentation and Kato Katz tests after each treatm… Show more

“…Gulhan and colleagues have also reported a case of a child in Turkey that was infected with F. hepatica and, did not respond to TCBZ treatment, although this report did not speculate as to the mode of infection with a potentially TCBZ‐resistant isolate. Similarly, Cabada and colleagues have reported a group of patients, infected by ingesting watercress in the Cusco region of Peru that failed to respond to multiple courses of triclabendazole. As with the Winkelhagen et al .…”

“…Cure rates of 95% using the 10mg/kg dose have recently been described in children, with 100% cure rates using two doses of 7.5mg/kg (120) . Reports of resistance to triclabendazole in livestock are increasingly common, and more recently likely resistance in human fascioliasis has been detected in the Netherlands and Peru (121,122) . Evidence from animal studies suggests triclabendazole resistance may be overcome by coadministration with the enzyme inhibitor ketonazole (123) .…”

“…Triclabendazole (TCBZ) is the drug of choice since its effective against juveniles and adult liver flukes, but resistance to this benzimidazole has emerged in livestock in different countries [11]. There have been recent reports of human fascioliasis refractive to TCBZ treatment in Peru and Chile [12, 13], highlighting a need for alternative drugs and treatments.…”
