Background: Several studies have focused on to find out the factors that may improve psychological, physical well-being, and quality of life among elders and prevent their abuse. Objectives: We investigated the impact of developing educational programs, using Intervention Mapping (IM) protocol and Kern Model in Primary Health Care Setting to prevent elder abuse. Methods: The integrated educational package was provided using both the IM protocol and Kern Model. The family members of the eighty elderly women were trained once a week for 4 sessions. The self-administrated Elder Abuse Questionnaire (EAQ) was completed before and after intervention by elderly women. The linear mixed model was used for the comparison of elder abuse frequency in two groups of control and trail. Results: The data of this study indicated that our intervention resulted in reduction in frequency of psychological and financial abuse (F=127.12, p<0.005; and F= 16.53, p<0.07 respectively) and neglect (F=95.4; p<0.005). None of the elders reported any physical abuse. Conclusion: This study showed that, the developed integrated educational package improves elder abuse and specifically it is tailored to the education of family members of the elderly women

Background and Aim: Caregivers in the home environment have an important role in timely detecting and responding to abuse. The aim of this review was to provide insight into both the existing tools for the assessment of and interventions for elder abuse by formal and informal caregivers in the home environment, and to categorize them according to a public health perspective, into primary, secondary, tertiary or quaternary prevention. Methods: We selected the assessment tools and interventions that can be used by caregivers in the home environment included in previous reviews by Gallione et al (2017) and Fearing et al (2017). To identify published studies after these reviews, a search was performed using PubMed, Cochrane Database, CINAHL and Web of Science. Results: In total, fifteen assessment tools and twelve interventions were included. The number of assessment tools for elder abuse for use in the home environment is increasing; however, tools must be validated over different cultures and risk groups. In addition, the tools lack attention for the needs of vulnerable older persons such as persons with dementia. Existing interventions for caregivers in the home environment lack evidence for addressing elder abuse and do not address potential adverse effects (quaternary prevention). Conclusion: Assessment tools for elder abuse need further testing for validity and reliability for use by caregivers in the home environment. For interventions, meaningful outcome measures are needed. Important to note is that quaternary prevention requires more attention. This argues for taking into account perspectives of (abused) older persons and caregivers in the development of assessment tools and interventions protocols.

Challenges for Detection of Elderly Abuse in Teaching Hospitals: A Interventional Study to Improve the Knowledge of Health Care Providers

Objectives: Elder abuse is a serious and dangerous form of domestic violence that is of particular concern to communities and healthcare professionals. Since health care providers have a central position for detecting elder abuse in hospitals, this study was conducted to determine the barriers in identifying elder abuse among health care providers and the effect of intervention to deal with the problem. Methods: The participants completed the health care providers' awareness on reporting the elder abuse questionnaire. Then, the interventional study was conducted on 235 health care providers at two teaching hospitals of Babol University of Medical Sciences. The intervention was conducted as a one-day workshop by two professors for four hours and included theoretical and practical subjects. Two months after implementing the protocol for detecting and screening elder abuse in hospital wards, the health care providers completed three questionnaires, including the awareness questionnaire, the potential barriers to assessing elder abuse in hospitals, and the health care providers' implementation problems in detecting and reporting elder abuse. Results: The mean health care providers' awareness to detect elder abuse improved significantly after intervention (8.51 ± 2.81 vs. 15.53 ± 2.39; P < 0.001). The challenges of abuse detection were; lack of awareness about elder abuse and its prevalence (54.8%), fear of legal consequences (51.9%), and fear of communication problems between older adults and their family (53.6%). Conclusions: More frequent and pragmatic education is necessary to strengthen the practical knowledge of health care providers in regards to the detection of elder abuse.
