Treatment and outcomes of arthrogryposis in the upper extremity

Abstract: Upper extremity involvement in patients with arthrogryposis multiplex congentia is quite frequent. Treatment initially consists of stretching and splinting as significant gains can be seen in the first years of life. The goal of any surgical procedure is to improve upper extremity function and performance of daily living activities, yet it is important to treat each patient individually and understand that areas do not always need to be addressed surgically. “…In brief, common surgical interventions for children with AMC include: (1) external rotation humeral osteotomies to improve position of the limb, elbow releases to improve ROM and the ability to self‐feed and; (2) carpal wedge and thumb reorientation osteotomies as well as webspace deepening to improve pinch, object manipulation, and object acquisition (Ezaki & Carter, ; Foy, Mills, Wheeler, Ezaki, & Oishi, ; Zlotolow, ; Zlotolow & Kozin, ; Zlotolow & Tiedeken, ). A detailed description on the orthopedic management of the upper limb can be found in another section of this special issue (Oishi et al, ).…”

“…In an effort to better characterize and define AMC, the first theme includes manuscripts representing an international effort to define AMC (Dahan‐Oliel, Cachecho, et al, ), updates on disease coding (Bedard & Lowry, ), classification (Hall, Kimber, & Dieterich, ), and gene ontology (Kiefer & Hall, ). The second theme spans several manuscripts describing advances for diagnosis and management, including guidelines for early detection (Filges, Tercanli, & Hall, ), diagnostic workup (Dieterich et al, ), rehabilitation (Wagner et al, ), and orthopedic management (Hamdy et al, ; Oishi et al, ). A standardized autopsy protocol is proposed by Oberg, Magaki, and Hall () to allow for a methodical approach for examination that will facilitate subsequent study by investigators across disciplines.…”
