Reading disorders are frequent in homonymous hemianopia and are termed hemianopic dyslexia (HD). The existing treatment methods have shown improvements in reading speed, accuracy, and eye movements during reading. Yet, little is known about the transfer effects of such treatments on functional, reading-related tasks of daily life, e.g., reading phone numbers, finding typing errors or text memory. In addition, little is known about the effects on symptom load and return to work. Here, we examined a new reading therapy entailing three different methods—floating text, rapid serial visual presentation (RSVP) of single words, and the moving window technique—and evaluated their efficacy. Twenty-seven chronic HD patients were treated in a baseline design with treatment-free intervals before and after a treatment period of several months. HD was assessed with a battery of reading tests and a questionnaire about subjective symptom load at four time-points. Patients received all three reading therapies over several weeks. The results show significant and stable improvements during treatment within all measures. Approximately 63% of treated patients returned to work after the therapy. We concluded that our novel HD treatment led to widespread and lasting improvements in reading performance, generalized to functional reading tasks and reduced symptom load, and the majority of patients were able to return to work.

IntroductionA significant number of Restitution Training (RT) paradigms claim to ameliorate visual field loss after stroke by re-activating neuronal connections in the residual visual cortex due to repeated bright light-stimulation at the border of the blind and intact fields. However, the effectiveness of RT has been considered controversial both in science and clinical practice for years. The main points of the controversy are (1) the reliability of perimetric results which may be affected by compensatory eye movements and (2) heterogeneous samples consisting of patients with visual field defects and/or visuospatial neglect.MethodsBy means of our newly developed and validated Virtual Reality goggles Salzburg Visual Field Trainer (SVFT) 16 stroke patients performed RT on a regular basis for 5 months. By means of our newly developed and validated Eye Tracking Based Visual Field Analysis (EFA), we conducted a first-time full eye-movement-controlled perimetric pre-post intervention study. Additionally, patients subjectively rated the size of their intact visual field.ResultsAnalysis showed that patients’ mean self-assessment of their subjective visual field size indicated statistically significant improvement while, in contrast, objective eye tracking controlled perimetric results revealed no statistically significant effect.DiscussionBright-light detection RT at the blind-field border solely induced a placebo effect and did not lead to training-induced neuroplasticity in the visual cortex of the type needed to ameliorate the visual field size of stroke patients.

The article considers the topical problem of neurology, neurobiology and cognitive psychology — the question of pathogenetic mechanisms of origin and development of unilateral neglect after a cerebral infarction. The paper analyzes the scientific literature on the neuropsychological signs of hemineglect syndrome, its causes, features, manifestations and methods of rehabilitation. The role of the laws of neuroaesthetics in the perception and reproduction of environmental information in the brain is shown. New data on the peculiarities of the influence of the laws of neuroaesthetics on the process of the unilateral neglect formation are presented. It is emphasized that the principles of neuroaesthetics in oxidative stress after vascular catastrophe of the brain create conditions for the development of the pathological circle, the manifestation of which is lack of attention and loss of ability to respond to stimuli in one half of the field of view. It is determined that the exact neuroanatomy of unilateral neglect is complex and remains the subject of future researches for further cognitive rehabilitation of patients after cerebral infarction.
