Traumatic Lumbar Hernias: Difficulty with Diagnosis and Repair

Abstract: Infrequently encountered, the traumatic lumbar hernia presents a challenge to the surgeon both in diagnosis and management. Repair is often complicated by a lack of viable fascia and a high incidence of associated injuries. The following review considers the natural history of lumbar hernias and their etiologies. Subsequently, the role of computed tomography (CT) in diagnosis is discussed. Finally, the multiple techniques of repair are presented, along with a discussion of the role of laparoscopy. Two case repo… Show more

“…1Y4 The literature at present consists of only a few small-scale reported series and a number of case reports. 5 Conflicting data exist to direct the treatment of these patients. 3,4 Less than 50% of traumatic lumbar hernias are clinically evident; therefore, a high index of suspicion is needed.…”

“…3,4 Less than 50% of traumatic lumbar hernias are clinically evident; therefore, a high index of suspicion is needed. 5,9 The approach to these patients is also changing with the increased use of laparoscopy. Few data exist to indicate when to perform the repair (or if repair is even needed) or whether to place a mesh (and the optimal type of meshV biologic vs. synthetic).…”

“…Incarceration has been reported in up to 25% of patients that do not receive treatment and strangulation in 10% – 18%. 7 , 13 , 14 However, these percentages were reported in articles published in the early 2000s. In our review, only 5% of delayed TLH presented as acutely complicated.…”

“…Robust evidence on diagnosis and treatment options is generally lacking. 7 The aim of our study is to systematically review the recent literature on TLH, focusing on the epidemiology, clinical presentation, diagnosis and treatment, with emphasis on surgical decision-making.…”
