Description
Mangled Extremiry Severity Score: MessThe Gustilo-Anderson classifi cation comprises the most commonly used classifi cation for open fractures of usually mangled extremities. However there are many discussions regarding the inter-observer reliability of this classifi cation system. Different studies show inter-observer reliability of approximately 60 % (ranging from 42 % to 92 %). Another important issue of Gustilo classifi cation is the inability to predict outcome. For this purpose, other classifi cation systems such as the Mangled Extremity Severity score (MESS) have been devised. The MESS score utilises four signifi cant criteria (with increasing points for worsening prognosis) including skeletal/soft-tissue injury, limb ischemia, shock, and patient age [ 1 -5 ].
Skeletal/Soft-Tissue InjuryLow energy (stab wounds, simple closed fractures, small-calibre gunshot wounds) 1 Medium energy (open or multiple-level fractures, dislocations, moderate crush injuries) 2 High energy (high speed MVA or high-velocity gunshot wounds) 3 Very high energy (high speed trauma + gross contamination) 4
