Traumatic Atlantoaxial Lateral Subluxation With Chronic Type II Odontoid Fracture: A Case Report

Abstract: Background: We report a case of an elderly patient who was diagnosed with lateral atlantoaxial subluxation with type II odontoid fracture, an extremely uncommon upper cervical spine injury that has not been previously reported in the literature to the knowledge of the authors. Methods: An 87-year-old male reported to the emergency room following a syncopal episode after sustaining a fall. He complained of dizziness, fatigue, disruption of vision and audition, and worsening neck pain. Computed tomographic scans… Show more

“…Traumatic AAD combined with type II odontoid fractures are even more rare, with only a few reports of such in the literature. [3] Our case is different from even these reports, in as much as the AAD was true lateral. In fact, our literature review uncovered only two reports of traumatic lateral AAD combined with type II odontoid fracture.…”

“…In fact, our literature review uncovered only two reports of traumatic lateral AAD combined with type II odontoid fracture. [ 3 , 4 ] Lenehan et al presented a case of lateral C1-C2 dislocation complicating a type II odontoid fracture treated with open reduction, C1 lateral mass screws, and C2 pedicle screws. [ 4 ] Musa et al also presented a case of traumatic atlantoaxial lateral subluxation with chronic type II odontoid fracture.…”

“…In addition, the atlas (C1) demonstrated an anterior displacement in relation to the C2, while the fracture was linked to the Type IV rotational atlantoaxial dislocation. These evaluations were based on Fielding classification ( 3 ).…”

“…This instigates the selection of closed reduction technique, where a halo head brace is used to provide traction, with a steadily increasing weight, up to 10% of the total bodyweight. Fielding and Hawkins ( 3 , 5 ) recommended < 9 kg for adults, and a successful procedure is progressed with a focus on the C2 fracture ( 2 ). However, a failed therapy, featuring poor displacement possibly requires additional conservative treatments with a Minerva brace for three months, which prompts quick reduction of C1-C2 dislocation.…”
