Transversus abdominis plane block: The new horizon for postoperative analgesia following abdominal surgery

Abstract: Post operative pain management is the key factor to decide the outcome of the patient. TAP block is relatively newer method for management of postoperative pain after abdominal surgery. Technique involves the injection of local anesthesia into the plane between the internal oblique and transversus abdominis muscle and thus giving pain relief. The technique when performed under ultrasound guidance improves the yield. TAP block provides good analgesia between T10 and L1 level hence very useful for lower abdomina… Show more

“…The studies have shown that TAP block provides significant analgesic effect, especially below T 10 up to L1 level; hence, it is perfectly suited for use after lower abdominal and gynecological surgeries [9] . Although most of the available studies on TAP block have used Local Anesthetic (LA) agent, few studies have reported that the adjuvant medications were added to LA to prolong the effect of TAP block.…”

“…It has got potential to substitute the use of intravenous opioid analgesics and hence to avoid its complications. It has been proved to cater significant analgesic effect especially below T10 up to L1 level [55][56][57][58][59].…”

“…The patient is positioned supine in both techniques [60]. There are two common techniques for TAP block [57][58][59][60][61]: (1) The blind TAP: the point of entry for the blind TAP is the lumber triangle of Petit. The needle is inserted cephalad to the iliac crest and advanced until two distinct "pops" are felt as the needle transverses the external oblique and internal oblique muscles.…”

“…TAP block is capable of giving good analgesic effect in the region between T10 and L1 following a single posterior infiltration (in midaxillary line) and to achieve higher block up to T6, it needs to be augmented with an anterior subcostal infiltration. 8 Various studies have been conducted for the postoperative analgesia in abdominal surgeries by comparing the TAP block with placebo or local wound infiltration. In this study, we compared postoperative analgesia in two groups by giving TAP block using ropivacaine and ropivacaine-dexmedetomidine (RD) combination.…”
