Objective: To evaluate the clinical and urodynamic impact of intravesical electrostimulation (IVES) on incomplete spinal cord injury (SCI) patients suffering from chronic neurogenic non-obstructive urinary retention (N-NOR). Methods: One-hundred and two patients underwent at least 28 consecutive daily IVES sessions because objective evidence of detrusor acontractility instead of hypocontractility was detected. Diary entries written at various stages by each patient were compared (7 days before the IVES cycle, 15-21 days into the cycle and 7 days before its end). Responders were patients with a mean 50% reduction in both the number of daily catheterizations and post-void residual urine. Responders underwent further urodynamics at the end of the IVES cycle; patients experiencing first sensation of bladder filling, and the mean volume of first sensation of bladder filling per ml, Qmax ml s À1 , among others, were evaluated. Nineteen individuals who repeated another IVES round were included in this study.Results: Thirty-eight subjects (37.2%) responded to IVES and of those, 83.3% recovered the first sensation of bladder filling after the IVES round. Nineteen responders repeated IVES within 1 year, owing to loss of efficacy. They obtained similar voiding symptoms improvement and urodynamic results as after the first IVES cycle. A timespan of o2 years from SCI to IVES, and the presence of first sensation of bladder filling at baseline represented significant predictive parameters for IVES success (Po0.05) using w 2 -test. Conclusions: IVES represents a possible therapeutic option for incomplete SCI patients with N-NOR.

Overactive bladder (OAB) is a highly prevalent disorder in the pediatric population. This condition is especially troublesome for pediatric patients and their families when associated with incontinence, since it negatively affects self-esteem and impairs children's development. From the patient's perspective, urgency and urge incontinence can have a significant impact, negatively affecting their quality of life. For a therapy to have true benefit, changes must not only be statistically significant, but must also be perceived as meaningful by the patient. A stepwise approach is favoured to treat this pathology, starting with behavioural therapy, followed by medical management, and eventually more invasive procedures.Antimuscarinic agents are the mainstay of medical treatment for OAB. Oxybutynin is the most commonly used antimuscarinic in the pediatric population. However, some patients have a suboptimal response to antimuscarinics and many experience bothersome side effects, which have been documented with all antimuscarinics to a significantly higher degree than placebo. Although there have been reports about the use of tolterodine, fesoterodine, trospium, propiverine, and solifenacin in children, to date, only oxybutynin has been officially approved for pediatric use by medical authorities in North America. This review will address alternative treatment options for pediatric patients presenting with OAB, from conservative measures to more invasive therapies.

Kratak sadržajUvod. Grupa znakova i simptoma koji se javljaju kao rezultat oštećenja nervnih korjenova caudae equinae smještenih u kičmenom kanalu nosi naziv sindrom kaude ekvine (cauda equina syndrome). Najčešće je uzrokovana lumbalnom hernijacijom intervertebralnog diska. Klinička slika se karakteriše bolom u krsnom, glutealnom i perinealnom regionu, bolom duž zadnje strane donjeg ekstremiteta, utrnulošću i motornim i senzornim deficitima na donjim ekstremitetima. Može se javiti i disfunkcija mokraćne bešike i potencije.Prikaz bolesnika. Pacijent dolazi na pregled kod fizijatra zbog jakog bola u slabinskom segmentu, koji se propagirao duž oba donja ekstremiteta praćen osjećajem slabosti, više lijevo, te nemogućnosti pražnjenja mokraćne bešike. Snimak dobijen nuklearnom magnetnom rezonancom (NMR) pokazuje protruziju intravertebralnog diska na dva nivoa. Pacijent je podvrgnut operativnom zahvatu: Hemilaminectomia L4 l. sin et foraminotomia L5-S1 l. sin, extirpatio disci. Mjesec dana nakon operativnog zahvata provodi postoperativnu stacionarnu rehabilitaciju. Na prijemu je detektovana slabost plantarnih fleksora lijevog stopala, te trnjenje u istom, nepotpuno pražnjenje bešike, nemogućnost kontrole spoljašnjeg analnog sfinktera i disfunkcija potencije. Nalaz urofloumetrije potvrdio je tešku hipotoniju detrusora mokraćne bešike koja graniči sa atonijom. Pacijent je proveo fizikalni tretman, te dva ciklusa elektrostimulacije detrusora mokraćne bešike. Kontrolni urodinamski nalaz bez značajnijih promjena u odnosu na prethodni. Nakon provedenog liječenja pacijent ima osjećaj olakšanog mokrenja, seksualna disfunkcija je manjа, snaga plantarnih fleksora je poboljšana.Zaključak. Na samom početku liječenja bili su prisutni faktori koji su ukazivali na teškoće u ishodu liječenja (hronični bol, odloženo operativno liječenje, odložena rehabilitacija). Smatramo da je poboljšanje nastupilo kao posljedica psihološkog stanja pacijenta koji uspijeva uticati na aktivnosti sfinktera.
