Aqueous-based nail lacquers have shown potential in promoting the diffusion of drugs into the nail. In our laboratory, we have recently developed a transungual delivery system based on an aqueous dispersion of cyclodextrin-poloxamer soluble polypseudorotaxanes, supramolecular host−guest assemblies that improves the drug permeation into the nail. However, the high-water content and the rheological and adhesive properties of this lacquer negatively affect properties that play a fundamental role in the patients’ acceptance such as stickiness, nail film formation or drying rate, properties. In this work, we have optimized the composition of these lacquers to improve these properties whilst maintaining good drug permeation profiles. Incorporating ethanol into the vehicle and reducing the proportion of Poloxamer 407 (PL), provided a good strategy. The use of hydro-ethanolic mixtures (>50% ethanol) and the reduction of the poloxamer concentration significantly improved the lacquer drying speed by reducing the stickiness and promoting film formation on the nail surface. Additionally, in a surprising way, the use of hydro-ethanolic vehicles further enhanced the permeation of ciclopirox olamine and clobetasol propionate, used for the treatment of onychomycosis and nail psoriasis respectively, into the nail and hooves.

Summary Background Onychomycosis is a fungal infection of the nails and is one of the most common dermatological infections worldwide. Infection occurs predominantly in the nails of the feet and if left untreated patients, particularly in at‐risk populations, can develop more serious complications, including pain, fissuring and secondary infections. Fungal infections are also contagious and may spread to other nails or to family members. Topical treatments can be effective for mild cases of the disease and typically have fewer contraindications. However, generation of robust efficacy data for topical therapies is often hindered by the difficulties associated with monitoring progression of mild onychomycosis using the conventional methods of mycological culture to obtain confirmation of podiatrist assessments. Objectives The aim of this study was to demonstrate image analysis as an effective method to monitor progression of fungal nail infections. Methods A novel digital image analysis technique was used to evaluate improvement in the visual signs of onychomycosis, when using a topical treatment kit for mild fungal nail infections in 60 participants over a 280‐day period. Results Image analysis demonstrated a significant (P < .0001) reduction from baseline of the affected nail area throughout the study period, results which were also perceived by the subjects themselves. Use of the treatment kit also limited the deterioration of the infection and prevented transmission to other nails. Conclusions Image analysis was established as a viable method to monitor the progression of fungal nail infections over long periods and demonstrate a clinical benefit following treatment.

Antifungal Nail Lacquer Loaded with Extract of Cissus quadrangularis for Treatment of Onychomycosis

Objectives: An attempt was made to prepare transparent nail lacquer containing natural antifungal agent obtained from extract of whole plant of Cissus quadrangularis for the treatment of onychomycosis. Methods: The extract of C. quadrangularis was evaluated for antifungal study against Candida albicans. Minimum inhibitory concentration of extract against C. albicans was performed to get the amount of extract to be loaded in the nail lacquer. Extract was further evaluated for phytochemical study such as test for steroids, glycoside and flavonoids. Nail lacquer was prepared by using nitrocellulose, ethyl cellulose, ethyl acetate, salicylic acid, dibutyl phthalate, extract of C. quadrangularis and acetone with continuous stirring. The nail lacquer of fluconazole was prepared and compared with formulation. Formulations of nail lacquer were evaluated for drying time, gloss, nonvolatile content, water resistance, viscosity, smoothness of flow. In-vitro transungual permeation study was performed through goat nail. Results: Phytoconstituents such as flavonoid and quinine were found present in the extract. Antifungal activity of nail lacquer and fluconazole formulation against C. albicans was found good (zone of inhibition of 20±2mm at 50mg/ml, 16±1mm for 40mg/ml respectively). Drying time of formulation was 62-70 s with visually seen glossiness of formulation. Formulation of nail lacquer showed good non-volatile content, water resistance and viscosity with smoothness of flow. In-vitro permeation of F7 formulation showed 90.9% of permeation which was faster within 24 hr. Conclusion: Thus, C. quadrangularis extract loaded antifungal nail lacquer could be a good choice for treatment of onchomycosis rather than fluconazole.
