The incidence of the various types of arrhythmia induced by defibrillation in anesthetized dogs that had had continuous ventricular fibrillation for 30 seconds was studied as a function of the current strength and the duration of the unidirectional rectangular pulse used to terminate the fibrillation. Shocks at 10, 20, 40, 80, and 100 amp with durations in the range of 80 fisec through 64 msec were used. At any given energy level the incidence of induced arrhythmias was much larger for the 40-, 80-, and 100-amp shocks than for the 10-and 20-amp shocks. At 10 and 20 amp, the incidence of induced arrhythmias increased with increasing energy content. Premature ventricular contractions were observed at nearly all levels of energy and current, in contrast to other types of arrhythmias that appear to be strongly related to energy content or current amplitude or both. В настоящей работе исследован частотный спектр импульсов, эффективность которых при дефибрилляции извест на. Рассмотрены импульсы: треугольные с крутым передним и пологим задним фронтом, импульс с пологим перед ним и крутым задним фронтом, биполярный асимметричный трапециидальный импульс дефибриллятора ДКИ Н 15 Ст БИФАЗИК+ и импульс обратный ему. Спектры импульсов вычислены и графически представлены с использо ванием программы Mathematica компании Wolfram Research Inc. Результаты. Показано, что различный характер зависимости эффективности дефибрилляции от длительности импульсов с инкрементом и декрементом затухания противоречит гипотезе о резонансном характере процесса электрической дефибрилляции сердца. Импульсы с оди наковым частотным спектром могут иметь существенную разницу эффективности при дефибрилляции. На эффек тивность может влиять интервал между фазами импульса. Заключение. Значение модуля спектральной характери стики при нулевой частоте биполярного импульса может рассматриваться как один из параметров, характеризующих эффективность и безопасность дефибриллирующего воздействия. Objective: to test the hypothesis of the resonance pattern of an electric cardiac defibrillation process and to study the impact of the frequency spectrum of pulses of various shape on the efficiency of defibrillation. Material and methods. The frequency spectrum of pulses the efficiency of which during defibrillation was known was examined. Delta pulses with a steep anterior and flat posterior front; a pulse with a flat anterior and steep posterior front, a bipolar asymmetric trapezoidal pulse from a DKI H 15 St BIFASIK+ defibrillator, and its returning pulse were considered. The pulse spectra were calcu lated and plotted using the Mathematica program (Wolfram Research Inc.). Results. The different pattern of a relationship of the efficiency of defibrillation to the duration of pulses with a damping increment or decrement is shown to be contra dictory to the hypothesis of the resonance pattern of an electric cardiac defibrillation process. The pulses with the equal frequency spectrum may have a great efficiency difference during defibrillation. The efficiency may be affected by the pulse phase interval. Conclusion. The value of a spectrum module at the zero frequency of pulse phases may be regarded as one of the parameters characterizing the efficiency and safety of defibrillation exposure.

Physical scientists and engineers have played important roles in helping to expand our understanding of the factors that influence the defibrillation process and in developing improved methods and devices for achieving cardiac ventricular defibrillation. The long-term experience of one engineering oriented group, based in a clinical department of a medical school, is summarized. Emphasized are the features of a series of research defibrillators that facilitated the generation of an extensive experimental database from studies in dogs and calves, the development of the first automatic implantable defibrillator to be successfully used in dogs, and studies that furnished the rationale for the widespread use of the uniphasic truncated exponential waveform and for the increasing interest in a variety of biphasic and multiphasic waveforms. Also considered are studies concerning the scaling of the defibrillatory shock with subject size and the role of compound units, defibrillation threshold, and contour graphs in the presentation and interpretation of data.
