While chronic lung disease causes substantial global morbidity and mortality, global estimates have primarily been based on broad assumptions. Specific country data from low-income countries such as Nepal are limited. This review assessed primary evidence on chronic respiratory disease burden among adults in Nepal. A systematic search was performed in June 2019 (updated May 2020) for studies through nine databases. High levels of heterogeneity deemed a narrative synthesis appropriate. Among 27 eligible studies identified, most were low-moderate quality with cross-sectional and retrospective study design. Chronic lung diseases identified were chronic obstructive pulmonary disease (COPD), asthma, bronchiectasis and restrictive lung diseases. Studies were categorised as: (i) community-based, (ii) hospital-based and (iii) comorbidity-related and disease burden. Reported disease prevalence varied widely (COPD, 1.67–14.3%; asthma, 4.2–8.9%). The prevalence of airflow obstruction was higher among rural dwellers (15.8%) and those exposed to household air pollution from domestic biomass burning as opposed to liquid petroleum gas users (Odds Ratio: 2.06). Several comorbidities, including hypertension and diabetes mellitus added to the disease burden. The review shows limited literature on lung disease burden in Nepal. Publications varied in terms of overall quality. Good quality research studies with prospective cohorts related to respiratory conditions are required.

Introduction: Chronic kidney disease (CKD) patients are at increased risk of morbidity and mortality because of their native disease progression. Accumulation of other atherosclerotic risk factors associated with CKD multiplies their risks. This study aimed to evaluate cardiac structural and functional abnormalities in CKD patients undergoing Maintenance Hemodialysis (MHD) using Electrocardiography (ECG) and Echocardiography (Echo). Method: It was a prospective cross-sectional study containing all CKD patients undergoing MHD at Pokhara Academy of Health Sciences, Pokhara, Nepal was conducted between August 2020 to December 2020. All the relevant data were entered in Statistical Package for Social Science (SPSS) for analysis. Results: There was a total of 115 patients with a male: female ratio of 1.8:1and a mean age of 47.25 ± 15.20years. The mean duration of dialysis was 36months and hypertension was the prime risk factor. Common ECG abnormalities were left ventricular hypertrophy, left ventricular strain pattern and poor R wave progression. Common Echo abnormalities were left ventricular diastolic dysfunction, left ventricular hypertrophy and regurgitation. Left ventricular systolic dysfunction was present in 20% of the patients. Conclusion: A spectrum of structural and functional cardiac abnormalities is common in CKD patients undergoing MHD. Early measures should be adopted to mitigate the risk.

Background and Aims: Unlike classically described polycythemia, anemia is found to be more prevalent in patients with chronic obstructive pulmonary disease (COPD).Anemia increases the cost of hospital stay and causes an increased risk of adverse outcomes including death in COPD patients. This study was done to find the prevalence of anemia in COPD patients, the factors associated, and the outcomes of anemic COPD.Methods: It was a quantitative, descriptive-analytical, and cross-sectional study conducted in Tribhuvan University Teaching Hospital's medical wards and the Emergency Room from September 2019 to September 2020. A simple random sampling method was used. Clinical information was obtained, and patients were followed up 3 months after discharge to document the number of exacerbations and deaths if present. Results:The patients in our study had a mean age of 70.80 ± 11.16 years. Most were female. Most (85.5%) had a history of exposure to firewood smoke. Twenty-three percent of the patients had anemia and these patients had significantly greater mortality 3 months postdischarge. Middle-old and old were more likely to have anemia with odds ratio (OR) of 2.55 (confidence interval [CI]: 0.48-13.5) and 13.6 (CI: 1.12-24.2), respectively. Current smokers had less likelihood of having anemia (OR: 0.05, CI: 0.006-0.49). Multivariate analysis showed that age, sex, and smoking status were significant determinants of anemia in COPD. There was no association between anemia and duration of hospital stay. However, mortality was higher at 3 months in COPD patients with anemia (p < 0.001). Conclusion:In COPD patients, anemia is prevalent comorbidity that is significantly linked to higher mortality but not to exacerbations. It is unknown, though, if treating anemia in COPD patients will affect the patient's outcome. Additional research in this area may be possible.
