Transstenotic Pressure Gradients: Measurement in Swine—Retrospectively ECG-gated 3D Phase-Contrast MR Angiography versus Endovascular Pressure-sensing Guidewires

Abstract: Phase-contrast MR angiography with VIPR enables reliable measurements of TSPG in carotid and iliac lesions that are comparable to those obtained with endovascular pressure-sensing guidewires. However, further work to compensate for respiratory motion is required to extend this technique to the renal arteries.

“…4D-flow measurement provides a noninvasive alternative. The advantage in comparison to pressure wire measurement is that not only vessel segments with an elevated pressure can be identified but the wall segment affected by the elevated pressure can also be visualized [23,24]. It must be taken into consideration that the absolute pressure at a certain location in the vessel cannot be specified with 4D-flow measurement but pressure gradients and their change over time can be measured.…”

“…Several validations of pressure gradient measurements in animals have been reported using PC VIPR (32,33). Figure 4 shows an example of a pressure gradient image across a carotid stenosis in a canine.…”

“…In a recent editorial commenting on the PC VIPR pressure gradient work of Lum et al (32), Prince (34) suggested that "In the future, our entire approach to vascular assessment may change, starting with analyzing pressure gradient maps. "…”

“…In 2007, Lum et al (33) retrospectively compared ECG-gated 3D PC VIPR versus endovascular pressure-sensing guidewires in swine models. The PC-derived trans-stenotic pressures provided reliable measurements of pressure gradients in a sample of mild to moderate carotid and iliac stenoses (r ¼ 0.952; P < 0.001).…”
