Transplants of Umbilical-Cord Blood or Bone Marrow from Unrelated Donors in Adults with Acute Leukemia

Abstract: Cord blood from an unrelated donor is an alternative source of hematopoietic stem cells for adults with acute leukemia who lack an HLA-matched bone marrow donor.

“…These results are consistent with previous reports [2,3,6,14,26,27]. The absence of significant increased incidence of acute and cGVHD after 9/10-HLA MMUD in comparison with 10/10-HLA MUD (data not shown) may be explained by a larger use of pre-transplant ATG in the MMUD group and of peripheral blood as a stem cell source in the MUD group.…”

“…However, we recently published a study conducted in acute myeloid leukemia patients over 50 which shows similar NRM and survival rates after unrelated donor and UCB transplants [28]. With high resolution HLA typing including HLA-DQB1 locus (10 digits), our results reinforce previously published results [2,3,6,[11][12][13][14]16,26,27,29], suggesting that UCB is an equally suitable stem cell source as MMUD, even if larger samples would have allowed for more powerful conclusions.…”

“…There is considerable evidence that CB is a promising option for patients who lack a human leukocyte antigen (HLA)‐matched related or unrelated donor 1. Several studies comparing results of CBT and either bone marrow or peripheral blood stem cell transplantation showed similar results regarding OS and leukemia‐free survival, despite slower hematopoietic recovery and higher incidence of GF for CB transplant recipients 2, 3…”

“…Current molecular techniques in combination with conventional cytogenetic analysis enabled us to firmly establish the donor-derived origin of MDS in this patient. In hemophilia A, up to 25% of new antifactor VIII (FVIII) inhibitory antibodies (inhibitors) occur in patients with mild or moderate disease [1]. Once the inhibitor develops, options for management include observation, immune modulation, and immune tolerance induction (ITI).…”

“…The use of a second unit helps ensure engraftment in larger adults and those with comorbidities, even though only one unit engrafts in most patients [1,2]. Herein, we present the clinical and laboratory characteristics of a patient who developed donor-derived myelodysplastic syndrome (ddMDS) after double umbilical cord-blood transplantation (dUCB HSCT).…”
