Objectives: This case report presents our experience regarding a horseshoe kidney from live donor to be used as a renal transplant. Materials and Methods:The recipient was a 48-year-old man with chronic renal failure owing to hypertension who had been on hemodialysis for 2 years. The donor was his 43-year-old sister who had an uncomplicated horseshoe kidney with negative results on a urinalysis. An aortogram showed that the arterial supply to the kidney consisted of 2 superior arteries (1 on each side) and 1 inferior accessory artery that was divided to feed the lower fused parenchyma of the kidney. Results: Surgery was performed via a retroperitoneal lumbotomy incision; the left half of the kidney was mobilized. The left kidney was procured by clamping the inferior accessory renal artery, transecting the parenchyma within the demarcation boundary. The transplant kidney was placed in the recipient's contralateral iliac fossa. The graft vein was anastomosed to the recipient's external iliac vein, the artery to the external iliac artery, and the ureter to the bladder. After perfusing the graft, no urine leakage was detected from the transacted surfaces, and the graft began producing urine. There were no complications after surgery. The patient was discharged on the 10th day after surgery with a creatinine level of 0.07 µmol/L. Maintenance immunosuppressive treatment included tacrolimus, mycophenolate mofetil, and prednisolone. Conclusions: We believe using a horseshoe kidney as a renal allograft after a detailed preoperative evaluation may help expand the donor pool.

Pasagos formos inkstas pasitaiko 1 iš 600-800 žmonių. Tokie inkstai pasižymi sudėtingesne anatomija, todėl jų transplantacija yra techniškai sunkesnė, tačiau įmanoma. Straipsnyje aprašoma, mūsų žiniomis, pirmoji pasagos formos inksto transplantacija Lietuvoje. Inkstas persodintas iš 38 metų vyro, mirusio dėl smegenų edemos, dviem recipientams -28 metų moteriai bei 53 metų vyrui, sergantiems 5 stadijos lėtine inkstų liga. Po operacijos abiem pacientams išsivystė ūminis transplantato nepakankamumas, tačiau po gydymo būkles pavyko stabilizuoti. Nors abu pacientai patyrė pooperacinių komplikacijų, negalima teigti, kad jos sietinos su inkstų anatomija. Dėl anatominių ypatumų pasagos formos inkstų transplantavimas yra susijęs su didesne komplikacijų rizika, todėl svarbu atidžiai atrinkti persodinimui tinkamus transplantatus ir pasirinkti geriausią operacijos metodą. Svarbu atkreipti dėmesį į inkstų vaskuliarizaciją, šlapimo kolektorinę sistemą, inkstus jungiančios sąsmaukos storį ir sandarą. aBStract Key words: horseshoe kidney, kidney transplantation. Horseshoe kidney is congenital anomaly that results from abnormal fusion at the lower poles (95 %), or the upper poles. It is found in approximately 1 in 600-800 persons. Because horseshoe kidneys have complex anatomy, it makes transplantation more difficult technically, but not impossible. We report the transplantation of horseshoe kidney, which to the best of our knowledge, is the first such transplantation in Lithuania. The donor was 38-year-old man, who died of cerebral edema. The kidneys were transplanted to two recipients: 28-year-old woman and 53-year-old man who both suffered from 5'th stage chronic renal disease. After the surgery both patients experienced acute graft failure, but after the treatment the grafts started to function normally. Although both patients suffered from post-surgical complications, the anatomy of the kidneys should not be named as the cause. Because of its anatomical features horseshoe kidney is associated with higher complication risk. Before the utilization of such kidney it is important to consider such factors as renal vasclar anomalies, urological anatomy and degree of fusion. These factors help decide what surgery method should be performed.

ÖzBöbrek transplantasyonu için donör havuzunun sınırlı olması nedeniyle, en sık görülen konjenital füzyon anomalilerinden olan at nalı böbrek; transplantasyon için önemli bir seçenek olabilir. At nalı böbrek; seçilmiş hastalarda transplantasyon için kullanılabilir ve tek bir alıcıya ya da bölündükten sonra iki ayrı alıcıya nakledilebilir. Kliniğimizde kadavradan elde edilen, Graves Tip 2 at nalı böbrek iki ayrı alıcıya başarı ile nakledilmiştir. Horseshoe kidneys may be used for transplantation to selected patients with a single recipient or divided and transplanted into to separate receivers. A Graves Type 2 horseshoe kidney obtained from a cadaver was successfully transplanted into two recipients in our clinic.
