Transmission probabilities of HIV and herpes simplex virus type 2, effect of male circumcision and interaction: a longitudinal study in a township of South Africa

Abstract: This study gave the first estimates of HSV-2 per-sex-act FtoMTPs in Africa. It demonstrated a synergy between HIV and HSV-2 infections and a protective effect of male circumcision on HSV-2 acquisition by males.

“…The balance of evidence suggests that male circumcision is protective against syphilis. [68][69][70] One meta-analysis with good evidence describes a protective effect (relative risk: 0.67 [95% CI: 0.54-0.83]), but there is considerable heterogeneity among the studies included. 68 An additional cohort study with fair evidence found that circumcised men were significantly less likely to have active syphilis at the point of study recruitment; when the men were followed up prospectively for 2 years, a protective effect was also observed but was nonsignificant.…”

“…HSV-2 is transmitted primarily through sexual contact and can cause ocular disease, long-term neonatal neurologic sequelae, or even mortality (1,2), resulting in a significant financial burden on health systems worldwide. HSV-2 infection can also increase the risk for HIV-1 acquisition by $3-fold (3,4). Despite intensive studies over the last two decades, an effective vaccine against HSV-2 has yet to be developed (5,6).…”

“…Limited preclinical research is available regarding the penile mode of transmission and an emphasis has been placed on vaginal and rectal transmission studies. The presence of foreskin has been associated with an approximately 50% increase in risk of acquisition of HIV [173][174][175]. The human penis, foreskin has potential HIV target cells such as CD4+ T cells, Langerhans cells, macrophages, and submucosal lymphoidal aggregates that are rich in CD3+ and CD4+ cells [176,177].…”
