Transjugular liver biopsy in patients with diffuse liver disease: comparison of three cores with one or two cores for accurate histological interpretation

Abstract: Three non-fragmented cores (each core >/=7mm in length) of TJLB can be considered a minimum requirement for histological assessment, giving better reproducibility in diagnosis as well as for inflammation and fibrosis.

“…23 We have shown that 3 passes results in a median length of 22 mm, CPT of 8, and PPT of 5, 23 and that the evaluation of 3 cores is always better than 1. 31 Microarchitectural variability at the liver periphery is greater than at deeper sites within the liver, 27 so this could favor assessing fibrosis in liver biopsy cores obtained by TJB, if sampling is adequate. There was no correlation between HVPG and biopsy length, so that sampling error was minimized and thus the pressure correlations described are far more reliable.…”

“…The diagnostic and prognostic value of transjugular liver biopsy is significantly improved when three individual, non-fragmented samples are submitted for analysis [70]. Standardization of liver biopsy assessment is essential for a uniform approach to the diagnosis and treatment of CLD and the acute insult.…”

“…So far, only few data have been reported evaluating mLLB as an alternative to TJLB in patients with severe coagulopathy [11]. If mLLB is not available a minimum of three needle passes with TJLB should be performed to obtain optimal tissue samples [24]. Cholongitas et al .…”

“…Cholongitas et al . did not find a difference in the number of portal tracts if at least three needle passes were applied in comparison with PLB using a Menghini needle [15, 24]. This group even suggested four needle passes in their most recent publication [25].…”
