Children with chronic long-term disorders need to move to the adult practice at some point in their life. Establishing a smooth and efficient transition process is a complicated task. Transition of medical care to adult practice is defined as the purposeful planned movement of adolescents and young adults with chronic physical and medical conditions from child-centered to adultoriented health care systems. This step is of the utmost importance for several reasons. There is an obvious deficiency of research in this area especially when it comes to pediatric inflammatory bowel disease (IBD). There is a considerable difference in individual practice among different centers. Also, age of transition varies among different countries and sometimes, even within the same country, transition age may vary among different provinces and districts! Interestingly, local politics and many factors other than children's welfare often play a role in deciding the age that older children move to adult practice at. This review discusses transition of children with IBD in view of the available evidence.

Approximately a quarter of patients with inflammatory bowel disease (IBD) are diagnosed before 20 years of age, presenting with more extensive distribution and severity of disease than adult onset. The purpose of this review was to determine facilitators of, and barriers to, successful transition of patients with IBD from adolescent to adult services. A systematic review of IBD transition research was conducted in March 2014 searching PubMed, MEDLINE, PsycINFO, Web of Science and the Cochrane Database of Systematic Reviews databases. A hand search of reference lists and narrative reviews was carried out to maximise the potential for retrieving all relevant manuscripts. Primary studies written in English of full-length peerreviewed journal articles that investigated transition of paediatric patients with IBD to adult services were included. Studies were excluded if the primary focus was not IBD. The search produced 283 potentially relevant studies. After removing duplicates and screening for suitability, six met our inclusion criteria. Barriers to transition included system inadequacies and a lack of resources, clinical time and training. Successful transition involved joint medical visits, structured transition services, improved communication between paediatric and adult services and improved education for patients and staff. If the transition process for adolescents suffering with IBD is to improve then it is vital that more research is conducted to better our understanding of ways in which we can ensure that defined protocols are in place for a smooth transition for every adolescent, leading to improved standards and minimal disruption to care. StreszczenieLeki immunosupresyjne mogą wywoływać poważne działania niepożądane. Opisano przypadek młodej pacjentki z wrzodziejącym zapaleniem jelita grubego (WZJG), u której po leczeniu azatiopryną wystąpiło ostre zapalenie trzustki. Stosowana przez 4 miesiące tioguanina oraz krótkotrwale (5 dni) cyklosporyna A były przyczyną pancytopenii, mononukleozy zakaźnej oraz opryszczkowego zapalenia skóry. Powikłaniom tym towarzyszyły zaburzenia nastroju w postaci zespołu lęko-wo-depresyjnego. Powrócono do klasycznej terapii mesalazyną podawaną łącznie z tianeptyną i uzyskano kilkumiesięczną remisję WZJG oraz poprawę stanu psychicznego. AbstractImmunosuppressive drugs can cause serious side effects. We describe a case of a young patient with ulcerative colitis (UC), in whom after azathioprine treatment acute pancreatitis occurred. Then for a period of four months tioguanine and briefly cyclosporine A (5 days) were given; these caused pancytopenia, infectious mononucleosis, and herpetiform dermatitis. Along with side effects, mood disorder as an anxietydepressive syndrome was present. Return to classical mesalazine therapy combined with tianeptine gave a few months of remission of UC and mental status improvement. WstępObraz kliniczny i przebieg wrzodziejącego zapalenia jelita grubego (WZJG) są zróżnicowane i uzależnione od wielu czynników, takich jak odmienność genetyczna, wiek pacjenta, choroby współistniejące, sposób i intensywność leczenia [1]. W lekkich postaciach stosuje się pochodne kwasu aminosalicylowego (5-ASA) oraz okresowo glikokortykosteroidy (GKS), ale ta metoda często nie spełnia oczekiwań. Zdarzają się przypadki zarówno steroidozależnosci i steroidooporności [2]. W cięższych postaciach choroby wprowadza się leczenie immunosupresyjne, a u części chorych wykorzystuje się przeciwciała monoklonalne przeciwko cytokinom prozapalnym [3]. Celem leczenia, niezależnie od sposobu, jest uzyskanie remisji klinicznej i wygojenie zmian zapalnych w bło-nie śluzowej jelita grubego.Wszystkie powyższe leki mogą wywoływać różno-rodne działania niepożądane. Dobór właściwej terapii wymaga dobrej znajomości mechanizmów działania zleconych leków oraz doświadczenia lekarza. Ważne są także właściwa współpraca lekarza z pacjentem oraz prawidłowe monitorowanie efektów terapii. Szczególny nadzór i wnikliwość lekarską należy wykazać, lecząc osoby młode, które przechodzą spod opieki pediatrów do poradni gastrologicznych dla dorosłych [4], czego przykładem jest przedstawiany przypadek uczennicy chorej na WZJG. Opis przypadkuPacjentka urodzona w 1991 roku w środowisku wiejskim, wzrost 161 cm, masa ciała 53 kg. Od wczesnych lat szkolnych skarżyła się na okresowe bóle głowy i brzucha. Nauka w szkole była dla niej czynnikiem stresującym, wywoływała stany podwyższonego napięcia emocjonalnego, uczucie wewnętrznego niepokoju i obniżenie nastroju. Od 14. roku życia nasiliły się bóle oraz wzdęcia
