Transient Optic Perineuritis as the Initial Presentation of Central Nervous System Involvement by Pre-B Cell Lymphocytic Leukemia

Abstract: A 20-year-old man with a history of pre-B cell acute lymphocytic leukemia (ALL) presented with optic perineuritis of the right eye while undergoing chemotherapy. Evaluation failed to reveal an infectious or neoplastic cause, and the patient improved with oral corticosteroid treatment. He returned 10 weeks later with complete loss of vision in the right eye. Optic nerve biopsy revealed leukemic infiltration of the optic nerve, and the patient was treated for central nervous system (CNS) relapse of ALL. Transien… Show more

“…The majority of cases are isolated and idiopathic [1,2]. Syphilis, Wegener's granulomatosis, sarcoidosis, Crohn disease and leukemia have been reported as causing optic perineuritis in adult patients [3][4][5][6][7][8]. Based on a PubMed literature search, limited information is available regarding optic perineuritis in pediatric patients.…”

“…It is a great mimicker of typical ON. OPN has been reported mostly as idiopathic and isolated cases [1] although associations with other diseases such as Wegener granulomatosis [2], giant cell arteritis [3], Crohn’s disease [4], Behcet disease [5], leukemia [6] and acute retinal necrosis [7] have been reported. To the best of our knowledge, there are limited reports of OPN in patients with NMO or NMOSD.…”

“…Similarly, in our series, 4 of 5 patients with leukemic infiltration of optic nerve achieved complete remission and had no previous history of central nervous system involvement. Due to their low prevalence, only a few literatures have addressed the MR imaging findings of leukemic infiltration of optic nerve (6,(8)(9)(10)(11)(12)(13). A search of the literature in English showed 6 cases of leukemic infiltrations of optic nerve (Table 3).…”

“…A search of the literature in English showed 6 cases of leukemic infiltrations of optic nerve (Table 3). MR imaging finding of the leukemic infiltration of optic nerve is thickening of the optic nerve with enhancement (6,(8)(9)(10)(11)(12)(13). Enhancing optic nerve thickening is also noted in patients with optic neuritis, however, no case of optic neuritis in leukemic patient has been reported yet.…”
