Transient Neurologic Deficits Associated With Carbamazepine-Induced Hypertension

Abstract: Carbamazepine is a well-established, effective treatment of complex partial seizures and is well tolerated in most patients. The adverse effects of carbamazepine include aplastic anemia, agranulocytosis, pancytopenia, bone marrow depression, thrombocytopenia, cardiac conduction abnormalities, congestive heart failure, and peripheral edema. Hypertension or hypotension has also rarely been documented in patients with either therapeutic or toxic blood levels of carbamazepine. It is possible that carbamazepine-ind… Show more

“…However, numerous reports exist in the medical literature of severe complications from transient spikes in HR or BP. 7 These include reports of cerebellar hemorrhage, 8 acute aortic dissection, 9 and transient neurologic deficits 10 in subjects with acute transient severe hypertension, as well as cardiomyopathy and heart block after physiologic tachycardia or tachycardia induced by cardiac stress testing. 11,12 Limitations of this study include possible patient selection bias.…”

“…Complications from transient spikes in HR and BP are rare and improbable; however, there are reports in the literature of complications, even from such transient increases . There are reports of brief neurologic events, cerebellar hemorrhage, cardiomyopathy, and acute aortic dissection …”

“…al. reported no significant complications. 9 Larger studies by Postma et al 2 and Cho et al 18 reported no major complications, and only two episodes of patients having vasovagal events out of 931 cases. A similar review of 443 IOLP, including use of the laser, by Koufman et al 10 showed only a 0.9% rate of complications.…”
