Transient global amnesia with post-hyperventilation temporal sharp waves—A case report

Abstract: We report a 55-year-old woman, who presented with transient amnesia for 7h following underwater swimming. There was no evidence of neurological disturbance except global amnesia. Sharp waves in both temporal regions were registered during the initial EEG recording, which was accentuated by hyperventilation. Right hippocampal high signal intensity was observed in DWI performed 56 h after symptom onset. The sharp waves were all disappeared on follow-up EEG recordings.

“…2,4 The exact pathogenesis of TGA is still unknown. 5 Although several etiopathophysiological mechanisms, such as migrainerelated mechanisms, hypoxic-ischemic events, 6 jugular vein valve incompetence, 7,8 and epilepsy-related activity, 9 have been argued to be associated with this syndrome, none of them seems to fully explain this condition.…”

“…This is most commonly described around cold water immersion (recreational, experimental or reason undocumented),20–24 as well a case documenting TGA following exposure to cryotherapy (extremely cold dry air),25 and a case while sailing in the rain 26. Another case is described of an individual experiencing TGA after swimming underwater, having held their breath for 1 min (intentionally, without near-drowning) 27. However, the nature and temperature of the water body are not disclosed and the case considered to be secondary to hypoxia by the authors.…”

“…With the termination of the clinical picture, there is a simultaneous decline of the non-specific EEG findings. 6 YooSeok Kwon et al, in their paper titled "'Left dominance of EEG abnormalities in patients with transient global amnesia"" published a table which presents the total electroencephalographic findings found by other researchers in patients with TGA. A number of 707 patients were studied.…”
