Transient elastography as a screening tool for liver fibrosis and cirrhosis in a community-based population aged over 45 years

Abstract: LSM proved to be a useful and specific procedure to screen for cirrhosis in the general population and to detect undiagnosed chronic liver disease in apparently healthy subjects.

“…KS of a normal human allograft was comparable to renal stiffness in 10-week-old landrace pigs without tubulointerstitial fibrosis and corresponded very well to previously published renal allograft stiffness of 32.7 kPa [17]. Thus, the regular KS even exceeds the stiffness of an advanced liver cirrhosis >20 kPa which is most likely because of the higher abundance of connective tissues and vascularization in kidneys under physiological conditions [25][26][27]. Considering the upper detection limit of the Fibroscan device (75 kPa), only a small measuring range from 30 to 75 kPa remains for the kidneys [8].…”

“…TE as a screening procedure in a general population > 45 years has been evaluated in two large studies, with a total of 4700 apparently healthy subjects [61,143]. One study found that the presence of diabetes mellitus and obesity was associated with a Young's modulus above 8 kPa.…”

“…Moreover, a recent study suggests that TE may be useful for screening of CLD (Roulot et al 2011). However, because of paucity of studies so far there is no recommendation yet on the possible use of TE as method of mass screening of CLD (Castera 2011).…”
