Abstract.The aim of the current study was to confirm the effect and elucidate the mechanism of bone marrow mesenchymal stem cells (BMSCs) in acute myocardial infarction (AMI). AMI was induced in mini-swine by ligating the left anterior descending coronary artery, and BMSCs (1x10 7 ) were injected via a sterile microinjection into the ischemic area. Six months postoperatively, electrocardiograph-gated single photon emission computed tomography revealed that the myocardial filling defect was reduced and the left ventricular ejection fraction was improved in the BMSC group compared with the control group (P<0.05). Histopathological examination indicated that, in the BMSC treatment group, the percentage of survived myocardial tissue and the vessel density were increased, and the percentage of apoptosis was decreased compared with controls (P<0.05). Reverse transcription-polymerase chain reaction results indicated that the expression levels of multiple inflammatory factors were significantly upregulated in the BMSC group compared with levels in the control group (P<0.05). In conclusion, the present study demonstrated that BMSC injection significantly improved cardiac function and reduced infarct size in six months, indicating that this method may be valuable for future study in clinical trials. IntroductionAccording to 2011 statistics, acute myocardial infarction (AMI) is one of the most prevalent causes of death and morbidity (1,2). Adult cardiomyocytes lack the capacity for regeneration, so scar tissue replaces lost myocardium following myocardial infarction, and this leads to cardiac remodeling with reduced left ventricular (LV) function (3). Cell therapy is a potential method for the regeneration and repair of myocardium and the improvement of myocardial function following ischemic injury (4). A number of cell types, including foetal Flk1 + CD34 + CD31 -bone marrow stem cells (BMSCs) have demonstrated cardiac regenerative capacity (5), however, the mechanism that stem cell transplantation utilizes in order to improve cardiac function following ischemic heart disease is unclear and there is little information available regarding the medium-and long-term effects. On the other hand, there exist a variety of transplantation methods, including local intramyocardial injection, intravenous injection, and intracoronary injection to the infarct zone (6). Intramyocardial injection of BMSCs has been demonstrated to be safe, and to lead to reverse remodeling and improved contractile ability of scarred areas (3,7). The mechanisms by which BMSCs reduce infarct size and improve cardiac function in animal models are complicated, involving engraftment, differentiation into functional cardiomyocytes and paracrine signaling (8,9). Cleft lip with or without cleft palate is the most common congenital anomaly in craniofacial complex, which is our target to deepen our understanding toward a molecular mechanism of dento-craniofacial morphogenesis. The hypothesis that the maternal effects in addition to the genetic ones play important roles on the dento-craniofacial morphogenesis in mice has been tested based on each developmental stage. Firstly, the maternal effects on the intrauterine craniofacial development in the mouse fetus were examined by means of embryo transfer technique, skeletal staining and cephalometry, indicating that the maternal effects were one of important factors on the intrauterine craniofacial morphogenesis of CL/Fr mice. Secondly, when the molecular nature of the maternal effects is elucidated, maternally derived growth factors may play important roles on mouse fetus development. Bone morphogenetic protein 4 (BMP4) and epidermal growth factor (EGF) function antagonistically, yet are coupled in the regulation of initial chondrogenesis. Smad1 serves as a point of convergence for the integration of two different growth factor signaling pathways during chondrogenesis in the mouse fetal mandible. Lastly, transforming growth factor-beta3 (TGF-b3) promoted fusion of cleft lip in the mouse fetus through the molecular pathways. In fact, microsurgical repair of cleft lip in the fetus that produced scarless fusion is mediated by TGF-b3 regulation of mesenchymal cell proliferation and migration at the site of repair. In addition, TGF-b3 promoted cell proliferation and angiogenesis in lip mesenchymal tissues. These events lead to enhancement of the lip fusion in the presence of TGF-b3. In neonatal mouse, application of exogenous TGF-b3 to decrease type I collagen accumulation and consequential scar formation provide the opportunities for the clinical augmentation of scar reduction after cleft lip repair. These findings indicate that the environmental factors provided by the dams cannot be ignored in the etiology of a craniofacial anomaly and/or development in the mouse model in addition to genetic ones. #

Aim: The purpose of this study was to assess the effectiveness of nasal spray midazolam by comparing it with conventional intravenous midazolam conscious sedation in minor oral surgical procedures.Objective: To assess the safety and effectiveness of the midazolam nasal spray in comparison to its IV midazolam. Patients and methods:Total forty number of patients were selected, and they were divided in to two groups. Group A for intra nasal midazolam atomized spray (n=20) and Group B for intravenous midazolam (n=20). Physiological parameters, anxiety score, sedation rating, patient's cooperation score, retrograde and anterograde amnesia were recorded for each patient during preoperative, intra operative and postoperative period. Final evaluation of safety and efficacy in the nasal and intravenous routes of midazolam drug during minor oral surgery were compared.Results: In the present study both intranasal and intravenous groups showed decrease in SBP and DBP intra operatively but in physiological limits and increase in the average pulse rates in the both groups. The average oxygen saturation levels were maintained to normal range in both the groups. The average respiratory rate decreased in the both intranasal and intravenous groups during surgical procedure. The preoperative to postoperative anxiety scores were decreased significantly in the both groups and there was no significant difference in pre to postoperative anxiety scores in between the groups. Conclusion:Both intravenous and intranasal routes of administration of midazolam showed patient cooperation, satisfaction and clinical effectiveness. Intranasal route of midazolam spray is effective in reduction of subjective stress feelings, reliable anxiolysis while preserving protective reflexes.
