Transesophageal echocardiography guidance for atrial-caval thrombus removal with the AngioVac system

Abstract: Thromboembolectomy is often guided with fluoroscopy. For intracardiac and great vessel thromboemboli, transesophageal echocardiography (TEE) can assess these thrombi, guide precise suction catheter placement, prevent intracardiac injury, and serve as a hemodynamic monitor. TEE can also be used to assess blood flow and thrombotic material reduction following embolectomy. TEE is a low-risk, high-value, real-time imaging modality that facilitates thromboembolectomy and increases patient safety.

“…The 11 AngioVac cases described in this study indicate that the procedure is an effective technique for removing vegetation or thrombus, particularly those that are soft and with minimal adhesion to adjacent tissue. Intraoperative TEE is a vital tool for assisting the procedure and evaluating intraoperative complications [ [5] , [6] , [7] , [8] ]. Fellowship-trained cardiac anesthesiologists who are NBE-certified in TEE are adequately qualified to provide TEE guidance during AngioVac procedures.…”

“…Such surveillance can decrease the possibility of collateral damage, including rupture of chordae tendineae and subsequent arrhythmia incitement. TEE monitoring also lessens the requirement for fluoroscopy, which lowers the dangers of ionizing radiation and contrast injection [ 8 ].…”

“…In our experience and as extensively described in the literature, echocardiographic assistance is commonly used in a number of emergency procedures, such as initiation and monitoring of ECMO, 37 assistance in placement of pulmonary artery catheters, 38 placement of intravenous pacemaker wires, 39 placement of intra-aortic balloon counterpulsation, 40,41 and during catheterdirected thrombolytic therapy. 42 Recent international guidelines recommend consideration of ECMO in patients with severe respiratory failure from SARS-CoV-2 infection when conventional management fails, if resources and expertise are available. 43 As of April 25, 2020, 563 patients with confirmed or suspected COVID-19 have undergone initiation of ECMO and are reported in the Extracorporeal Life Support Organization registry.…”
