Transendocardial delivery of autologous bone marrow enhances collateral perfusion and regional function in pigs with chronic experimental myocardial ischemia

Abstract: Bone marrow cells secrete angiogenic factors that induce endothelial cell proliferation and, when injected transendocardially, augment collateral perfusion and myocardial function in ischemic myocardium.

“…The mechanism underlying improvement of systolic and diastolic function after bone marrow cell transplantation could be related to promotion of angiogenesis, as proposed by both preclinical and clinical studies (2,5,6,15). Patients in the current study had advanced coronary artery disease and stress-inducible myocardial ischemia, resulting in mild diastolic dysfunction.…”

“…In preclinical models, cardiovascular stem cell therapy has been used successfully in a variety of cardiac diseases, including dilated cardiomyopathy (4)(5)(6)(7)(8)(9). In the clinical setting, cell therapies have been previously used in patients with ischemic heart disease, both in the acute and chronic stages.…”

“…Collateral flow at rest and during adenosine stress increases, accompanied by as much as a 50% increase in regional myocardial wall thickening during stress. 10 Based on these reports, we might speculate that similar results could be obtained by using the intrinsic reparative mechanisms of either homing or mobilization. These approaches have also been successful in animal laboratory studies.…”
