The objective of the study was to determine the risk factors for development of circumferential fistula. We carried out a crosssectional, multicentric and analytical study over 7 years period, from 1st January, 2010 to 31 December, 2016. We compared circumferential and non- circumferential fistula patients in order to determine the risk factors for circumferential fistula development. Circumferential fistula accounted for 20% (91/456) of all vesico-vaginal fistulas. The mean age of the 456 patients was 35.9 years±12.15 (min 15 years; max 72 years). On univariate analysis, factors associated with the risk of circumferential fistula were: residence (P=0.039; OR=1.7), parity (P=0.04; OR=0.47), marital status before fistula (P=0.002; 4.3), duration of labor (P=0.041; OR=2.7) and fistula aetiology (P=0.038; OR=2.54). In a logistic regression model, two factors remained significant: marital status before fistula (P=0.029; OR=0.13) and duration of labor (P=0.017; OR=0.26). Circumferential fistula occurs in urban, primiparous, unmarried women who have been in labor for more than 41 hours.

There are about 200 million people on the move in the world, and approximately 50% of them are women. There are no clear migration plans for women leaving as a result of persecution, war, famine, climatic disasters or moving away from contexts of external abuse and even intrafamily violence. Gender-related violence, to which women are exposed in cultural contexts characterized by a patriarchal social organization, is manifested through different ways including, but not limited to, early marriages and genital mutilation, with reproductive health already being seriously impaired at an early age. To this must be added the consideration that low-income countries are not able to deal with chronic degenerative diseases with a multidisciplinary approach such as diabetes. Fragile or non-existent health systems are not prepared for this need, which now affects a third of all deaths from this cause. Compared to Italian mothers, women from high-migration pressure countries had a higher risk of gestational diabetes; in addition, young women of Ethiopian ethnicity are more exposed to increased diabetes risk, in an age- and BMI-dependent way. Gender inequalities are also more evident in migrants for other non-communicable diseases besides diabetes. A major effort is needed in terms of training practitioners and reorganization of basic health services, making them competent in an intercultural sense. Health education of the population as a whole and of women specifically is also needed to contain risk behavior and prevent the early onset of metabolic syndromes in general and of type 2 diabetes in particular.

Introduction Female genital fistula remains a global concern especially in incoming countries. The majority of cases resulted from obstructed delivery labor. The treatment is by surgery exceptional by probe for very small cases. But some ones remain incurable after several surgery theaters. The objective of this study is to establish the epidemiology anatomopathological types and prognosis of FGF considered being incurable in our center. Methods This is a retrospective and prospective descriptive study from January 2, 2017 to July 14, 2021 on incurable cases of female genital fistula recorded at the Centre National de Réparation de Fistule Obstétricale (CNRFO) of Niamey for complex cases of fgf. The epidemiology, the lesions of vagina, uretra, bladder, the treatment and prognosis have been studied. Results A total of 522 cases were operated within 5 cases given incurable (0.95%). The 5 cases were 37 -49 years old, they had fistula at their first delivery, the mean age of the fistula was around 9 years. The anatomopathological founds were vaginal severe sclerosis, extensive supra-trigonal fistula, total ureteral destruction, and partial bladder destruction. All the 5 patients treated by surgery were declared incurable. Conclusion Female genital fistula is not life-threatening but a major definitive handicap for theses prim parous. The treatment fail is linked with local lesions, continuous urine and fecal incontinency, added by iterative surgery. They remain judged incurable whether a surgery was by skilled practitioner.
