Purpose The prevalence of non-specific posterior neck pain (NPNP) has been increasing yearly. There are several treatments for NPNP, but in this review, we will focus on thread-embedded acupuncture (TEA). TEA is used in the Republic of Korea, Taiwan, and China for managing musculoskeletal diseases, obesity, and sequelae of facial palsy. However, there is insufficient evidence on its effectiveness and safety. This study aims to assess the efficacy and safety of TEA for NPNP. Methods We will search the following nine electronic databases, from their inception to May 2020: MEDLINE, Embase, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure, Citation Information by the National Information infrastructure, Korean Studies Information Service System, National Digital Science Library, Oriental Medicine Advanced Searching Integrated System, and Research Information Service System. Only randomized controlled trials of TEA for NPNP will be included. The methodological quality of the included trials will be assessed using the Cochrane risk-of bias tool. To conduct the meta-analysis, the risk ratio and mean difference with the 95% confidence interval will be used. Sensitivity analyses will be conducted based on this protocol. Results The results of this study will be submitted to a peer-reviewed journal for publication. Conclusion The results of this study would provide the evidence of whether TEA can be effective for treating NPNP. Registration Number PROSPERO CRD42020188346.

Manual acupuncture (MA) can be used to manage high blood pressure, however, the underlying molecular mechanism remains unknown. To explore the mechanism of acupuncture in the treatment of hypertension, we used Wistar Kyoto rats (WKYs) and spontaneously hypertensive rats (SHRs) that were subject to either MA stimulation or the corresponding sham procedure as a negative control (sham-MA) for 1 week. Blood pressure was recorded regularly. After 7 days of treatment, PET-CT scans were used to detect brain areas where glucose metabolism was significantly regulated. Additionally, the differentially expressed genes(DEGs) of a specific brain region—the paraventricular hypothalamic nucleus (PVH)—were evaluated by transcriptomics, and verified with quantitative PCR (qPCR). Eight overlapping DEGs were found between the WKY, SHR, and MA groups. The DEGs were then annotated with the Gene Ontology (GO) and the Kyoto Encyclopedia of Genes and Genomes (KEGG) databases. qPCR was used to verify the DEGs. These genes may lower blood pressure by regulating angiotensin, endothelial function and inflammation, ect. This study reveals MA regulates multiple biological processes and genes of the PVH, and provides a solid theoretical basis for exploring the mechanisms by which MA manages hypertension.

Background Thread embedding acupuncture (TEA) has recently been used as a conservative treatment method for non-specific chronic neck pain (CNP). The objective of this study was to evaluate the effectiveness and safety of TEA compared to physical therapy (PT) for treating patients with CNP. Methods A total of 128 patients diagnosed with CNP were randomly assigned to a TEA group and a PT group at a 1:1 ratio. The TEA group received four sessions of TEA, while the PT group received eight sessions of PT over 4 weeks. Outcomes were assessed using Neck Pain and Disability Scale (NPDS), clinically important difference (CID), cervical spinal angle, Beck Depression Inventory II (BDI-II), Beck Anxiety Inventory (BAI), Patient Global Impression of Change (PGIC), and EuroQol Five-Dimension (EQ-5D) at baseline and 5, 9, and 13 weeks. Results The TEA group showed significant improvement in NPDS compared to the PT group at 5, 9, and 13 weeks. Proportions of patients with decreased NPDS scores of more than 11.5 points (minimal CID) were significantly higher in the TEA group at 5, 9, and 13 weeks. There were significant differences between the two groups at 5, 9, and 13 weeks for BDI-II, and at 5 and 9 weeks for BAI. For EQ-5D, the TEA group showed significant improvement at 5, 9, and 13 weeks. There was no significant difference in cervical spinal angle between the two groups. For PGIC, better improvement was observed at 9 and 13 weeks in the TEA group. Adverse events associated with interventions were mostly temporary and mild. Conclusion For patients with CNP, TEA treatment was found to be more effective than PT treatment for improving their pain and dysfunction, quality of life, and psychological distress. Despite some post-treatment discomfort, TEA treatment can be considered as a useful treatment method for patients with CNP. Trial Registration This trial has been registered 5 April 2019 in Clinical Research Information Service of South Korea (CRIS- KCT0003720).
