Transcatheter sapien valve implantation in a native tricuspid valve after failed surgical repair

Abstract: Transcatheter Sapien valve implantation in a native tricuspid valve after failed multiple surgical repairs is feasible by the femoral vein. Technical challenges due to the lack of rigid landing zone and fluoroscopic markers were solved by prestenting and valve implantation under bi-plane fluoroscopic and TEE guidance.

“…Percutaneous tricuspid valve replacement (TVR) can be a feasible treatment for high‐risk patients with severe symptomatic TR that recurs after surgery . There is limited experience with percutaneous interventions for this valve, with small case series of percutaneous TVR on patients with prior surgical TVR or conduits , and few case reports on patients with prior tricuspid valve repair .…”

“…We were able to find only one report of Edwards Sapien valve implantation into a native tricuspid valve; in this case, pre-stenting techniques were used to establish the rigid target zones that are typically needed for successful transcatheter valve implantation. 7 Large registry studies are necessary to further evaluate the usefulness of this procedure. Long-term followup evaluation will ultimately determine the safety and durability of transvenous transcatheter valve-in-valve implantation after bioprosthetic tricuspid valve failure.…”

“…The bioprosthetic valve frame or annuloplasty ring serve as a rigid and fluoroscopically visible landing zone for the new transcatheter valve. We are aware of just a single previous case report of a transcatheter Sapien valve implantation in a native tricuspid valve following multiple failed surgical repairs where an annuloplasty ring was not present . In this report, we describe a second case where a Melody valve was placed in the tricuspid position in a native tricuspid valve following multiple failed surgical repairs without the presence of a supporting annuloplasty ring.…”
