Transcatheter recanalisation and stenting of a closed ductus arteriosus in duct dependent lung perfusion

Abstract: We present the case of a 64 year old man, previously of good health, who presented in November 1995 with a short history of presyncope and one witnessed episode of syncope. On admission he was clinically in complete heart block with a ventricular rate of 40 beats/min but had a well maintained blood pressure at 150/80 mm Hg. Auscultation revealed a soft systolic murmur in the aortic area. The remainder of the examination was unremarkable. Resting ECG confirmed complete AV block with a slow regular broad complex… Show more

“…Postnatal patency of the arterial duct in congenital heart disease with duct-dependent pulmonary circulation is life saving 13 8. Maintaining the patency by stent implantation has been proposed as an effective alternative to a surgical systemic-to-pulmonary artery shunt in neonates who are unsuitable for primary repair or in whom there is expected spontaneous improvement of oxygen saturation as the pulmonary vascular resistance decreases.…”

“…Cannulation of the ductus by the retrograde approach from descending aorta is rarely successful. Antegrade cannulation of the left AD in patients with right‐sided aortic arch has been shown earlier [3, 4]. We describe a technique of antegrade cannulation of the vertically arising AD in patients with pulmonary atresia and VSD that was consistently successful and reproducible.…”

“…Transcatheter recanalization of peripheral and central veins, arteries, or surgically created shunts or baffles has been performed in selected patients and conditions in the past [1–6]. Such vessel occlusions are related to luminal thrombosis or intimal proliferation.…”
