Transcatheter Arterial Embolization Therapy for Huge Renal Cysts: Two Case Reports

Abstract: We encountered 2 patients with symptomatic huge simple renal cysts. In case 1, 4,000 mL of cyst fluid was drained via a catheter, but intracystic bleeding occurred immediately afterwards. Transcatheter arterial embolization (TAE) was performed, after which the bleeding stopped, and cyst drainage was repeated successfully. After 2 years, the total cyst volume was reduced from 11,775 mL to 75.4 mL. In case 2, TAE was performed prophylactically before drainage. Subsequently, 9,400 mL of fluid was removed from mul… Show more

“…1 There are scarce case reports in the literature discussing the treatment of intracystic bleeding with transcatheter arterial embolisation, as was done with this patient. 2 Our patient's bleeding was well controlled with embolisation, which prevented the need to progress to surgery.…”

“…Toriu et al [3] stated that their 2 reported cases had illustrated 2 main clinical issues including: (1) per-cutaneous catheter drainage of huge cysts is associated with a risk of severe intra-cystic haemorrhage, and (2) the undertaking of prophylactic TAE can be undertaken to prevent the development of such haemorrhage. Shiraishi et al [21] undertook a Japanese retrospective study related to the undertaking of laparoscopic decortication of kidney cysts of 36 patients following which they suggested that laparoscopic decortication of kidney cysts is a safe procedure that tends to be associated with satisfactory long-term symptomatic results.…”

“…Nevertheless, if a simple cyst of the kidney becomes symptomatic, it does need to be treated and percutaneous drainage of the kidney cyst combined with sclerotherapy or surgical treatment tends to be selected [2]. However, it has also been iterated that complex cysts of the kidney tend to be treated surgically in view of the fact that complex cysts of the kidney tend to be associated with a high incidence of malignancy [2,3,4]. Apart from aspiration and drainage of simple kidney cysts, treatment of simple kidney cysts that are large and symptomatic or have been associated with haemorrhage into the cyst can be treated by means of selective angiography an super-selective embolization of the renal arterial branch that supplies the blood to the kidney cyst.…”
