Transcanal Endoscopic Ear Surgery for the Management of Congenital Ossicular Fixation

Abstract: The transcanal endoscopic approach was successful in improving hearing in pediatric patients with congenital ossicular fixation that involves any of the three ossicles. An endoscopic transcanal approach provides an alternative method to manage congenital ossicular pathology with the advantage of providing excellent visualization and the avoidance of a postauricular incision.

“…Proponents of endoscopic approach for primary stapes surgery cite enhanced visualization of the oval window and ossicles, with resultant improvement in diagnostic accuracy, as a primary advantage of the approach . These same advantages apply to revision stapes surgery for surgeons already routinely performing totally endoscopic stapes surgery.…”

“…An endoscopic approach to stapes surgery allows for enhanced visualization of the ossicular chain and oval window . Additionally, endoscopy of the middle ear has been especially useful in diagnosing ossicular malformations, perilymphatic fistulas, and ossicular fixation …”

“…The number of subjects collected for meta‐analysis was 1004, including 513 patients belonging to EES and 491 patients under MES, and the included studies were published between 2015 and 2017. Nineteen studies were analyzed qualitatively, which were all case series studies, except for one retrospective cohort study (Table ) . The included studies were published between 1995 and 2017, and a total of 406 patients were evaluated.…”

“…1,2 Although endoscopic sinus surgery has become a standard technique, endoscopic ear surgery has developed relatively late due to associated disadvantages, such as one-hand surgery, lack of depth, spatial limits of the ear canal, and potential of heat injury from the instruments. 1,3,4 However, recent advances, such as endoscope holders and increased resolution of endoscopic cameras, have overcome many technical problems. Moreover, successful outcomes of endoscopic ear surgery have been reported and compared to those of conventional surgery using a microscope.…”

“…Moreover, successful outcomes of endoscopic ear surgery have been reported and compared to those of conventional surgery using a microscope. 4 Minimal invasiveness being one of the important requirements in almost all surgical fields, the demand for endoscopic ear surgery is expected to increase significantly in the future.…”
